摘　　要：目的分析降钙素原(PCT)、超敏C-反应蛋白(hs-CRP)、N末端脑钠肽前体(NT-proBNP)检测在脓毒症诊断中的应用效果。方法选取2019年1月-2020年12月期间医院接收的80例脓毒症患者,另选取80例一般感染患者及80例非感染患者纳入研究,检测3组PCT、hs-CRP、NT-proBNP水平,以序贯器官功能衰竭评分(SOFA)为标准,评价3种指标检测对脓毒症的诊断效能。结果脓毒症组PCT、hs-CRP、NT-proBNP水平高于一般感染组、非感染组,而一般感染组PCT、hs-CRP、NT-proBNP水平高于非感染组,差异有统计学意义(P<0.05);PCT、hs-CRP、NTproBNP检测对脓毒症的诊断灵敏度为82.50%、76.25%、71.25%,特异度为85.00%、77.50%、80.00%,阳性预测值为73.33%、62.89%、64.04%,阴性预测值为90.67%、86.71%、84.77%,约登指数为0.68、0.54、0.51。结论 PCT、hs-CRP、NT-proBNP检测有助于诊断脓毒症,指导早期开展治疗工作,避免病情恶化。Objective To analyze the application effect of procalcitonin(PCT),high sensitivity C-reactive protein(hs-CRP)and N-terminal pro-brain natriuretic peptide(NT-proBNP)in the diagnosis of sepsis.Methods 80 patients with sepsis received from January 2019 to December 2020,and 80 patients with common infection and 80 patients with non-infection were included in the study.The levels of PCT,hs-CRP and NT-proBNP in three groups were detected,and the diagnostic efficacy of the three indicators for sepsis was evaluated with the Sequential Organ Failure Score(SOFA)as the standard.Results The levels of PCT,hs-CRP and NT-proBNP in sepsis group were higher than those in general infection group and non-infection group,while those in general infection group were higher than those in non-infection group(P<0.05).The diagnostic sensitivity of PCT,hs-CRP and NT-proBNP to sepsis was 82.50%,76.25%and 71.25%,The specificity is 85.00%,77.50%,80.00%,The positive predictive values were 73.33%,62.89%,64.04%,The negative predictive values were 90.67%,86.71%,84.77%,The Jordan index is 0.68,0.54 and 0.51.Conclusion The detection of PCT,hs-CRP and NT-proBNP is helpful for the diagnosis of sepsis,guiding the early clinical treatment,avoiding the deterioration of the disease.
