64排128层螺旋CT血流动力学参数对胃癌患者淋巴结转移的评估分析

摘　　要：目的分析64排128层螺旋CT血流动力学参数对胃癌患者淋巴结转移的评估价值。方法选取2019年1月至2020年12月长葛市人民医院接收的100例胃癌患者,均采用64排128层螺旋CT行体部灌注扫描序列进行检查,记录主要血流参数[血容量(BV)、血流量(BF)、表面通透性(PS)、平均通过时间(MTT)],并依据手术病理结果统计其淋巴结转移情况,对比伴淋巴结转移与未伴淋巴结转移胃癌患者主要血流参数,绘制受试者工作(ROC)曲线分析64排128层螺旋CT对胃癌患者淋巴结转移的评估价值。结果所有患者中,有62例伴淋巴结转移,38例未伴淋巴结转移;伴淋巴结转移与未伴淋巴结转移胃癌患者性别、年龄、合并症比较,差异无统计学意义(P>0.05);伴淋巴结转移胃癌患者BF、PS值均高于未伴淋巴结转移胃癌患者(P<0.05);组间BV、MTT值比较,差异无统计学意义(P>0.05);ROC曲线显示,BF、PS单独及联合预测胃癌患者伴淋巴结转移风险的曲下面积均>0.80,预测价值较理想,且以联合预测价值最佳。结论提示64排128层螺旋CT血流动力学参数对胃癌患者淋巴结转移的具有一定诊断价值。Objective To analyze the value of hemodynamic parameters of 64 line 128 layer spiral CT in the evaluation of lymph node metastasis in patients with gastric cancer.Methods One hundred patients with gastric cancer received by Changge People’s Hospital from January 2019 to December 2020 were selected.All patients were examined by 64 line 128 layer spiral CT for scanning the body perfusion sequence,the main blood flow parameters[blood volume(BV),blood flow(BF),surface permeability(PS),mean transit time(MTT)]were recorded,and the lymph node metastasis was analyzed according to the pathological results.The main blood flow parameters of gastric cancer patients with and without lymph node metastasis were compared.Receiver operating(ROC)curves were drawn to analyze the value of 64 line 128 layer spiral CT in evaluating lymph node metastasis in patients with gastric cancer.Results Among the 100 included patients,62 patients had lymph node metastasis and 38 patients did not have lymph node metastasis.There was no statistical significant difference in gender,age and complications between patients with and without lymph node metastasis(P>0.05).The BF and PS values of gastric cancer patients with lymph node metastasis were higher than those of gastric cancer patients without lymph node metastasis(P<0.05).There was no statistical significant difference in BV and MTT values between the two groups(P>0.05).The ROC curve showed that the area under curvature of BF and PS alone and combined to predict the risk of lymph node metastasis in gastric cancer patients were all>0.80.The prediction value was ideal,and the joint prediction value was the best.Conclusion Hemodynamic parameters of 64 line 128 layer spiral CT has certain diagnostic value for lymph node metastasis in patients with gastric cancer.
