医院门诊处方超说明书适应证范围用药分析

摘　　要：目的对本院门诊超说明书适应证用药的处方进行分析讨论。方法统计本院处方点评系统中1375张不合理处方,抽取其中超说明书适应证范围处方61张进行分析讨论。观察超说明书适应证范围处方的科室分类并对超说明书适应证范围处方进行汇总。结果61张超说明书适应证范围处方中,内分泌科占比57.38%(35/61),其次为口腔科占比32.79%(20/61),皮肤科和妇产科均占比4.92%(3/61)。超说明书适应证范围处方用药主要包括7种,其中用于Ⅱ型糖尿病辅助治疗的黄连素片占比最高(39.34%),其次是外用治疗糖尿病足的盐酸山莨菪碱注射液(18.03%),碳酸氢钠片、沙利度胺片、羟氯喹片分别占比14.75%、9.84%、8.20%,螺内酯片和二甲双胍片均占比4.92%。结论依据《处方管理办法》,《医院处方点评管理规范(试行)》对超说明书适应证范围的处方应加强管理,能够提高处方质量,保障患者用药安全合理。Objective To analyze and discuss the prescriptions beyond the indications of drug instructions in hospital outpatient.Methods 1375 unreasonable prescriptions in the prescription review system of our hospital were counted,and 61 prescriptions beyond the indications of drug instructions were selected for analysis and discussion.The Department classification of prescriptions beyond the indications of drug instructions was observed,and prescriptions beyond the indications of drug instructions were summarized.Results Among 61 prescriptions beyond the indications of drug instructions,the endocrinology department accounted for 57.38%(35/61),followed by the stomatology department for 32.79%(20/61),and the dermatology department and obstetrics and gynecology department both accounted for 4.92%(3/61).The prescriptions beyond the indications of drug instructions mainly included 7 kinds of drugs,of which berberine hydrochloride tablets for the adjuvant treatment of type 2 diabetes mellitus accounted for the highest proportion(39.34%),followed by anisodamine hydrochloride injection(18.03%)for topical treatment of diabetic foot,sodium bicarbonate,thalidomide and hydroxychloroquine tablets accounted for 14.75%,9.84%and 8.20%,respectively,and spironolactone tablets and metformin tablets both accounted for 4.92%.Conclusion According to the"prescription management measures"and"hospital prescription review management standard(Trial)",we should strengthen the management of prescriptions beyond the indications of drug instructions,improve the quality of prescriptions,and ensure the safety and rationality of drug use of patients.
