胆胃舒颗粒预防ERCP术后胆总管结石复发远期疗效分析

摘　　要：目的观察胆胃舒颗粒预防内镜逆行胰胆管造影(ERCP)术后胆总管结石复发的远期疗效。方法回顾性分析我院193例行ERCP胆总管取石术患者的临床资料,通过COX回归模型对术后是否给予中药预防、性别、年龄、胆总管直径、结石大小、结石数量、憩室、胆总管十二肠瘘、EST切开大小、乳头球囊扩张、术中碎石等11个可能影响胆总管结石复发的因素进行分析。结果ERCP术后6~10年期间,中药预防组和对照组累计胆总管结石复发概率分别是8.5%和19.1%,差异具统计学意义(P<0.05)。其中患者术后是否中药预防、结石数量、胆总管直径、术中碎石因素对术后复发有显著统计学意义(P<0.05);患者的年龄、性别、胆总管结石大小、憩室、胆总管肠瘘、EST小/中切开、乳头球囊扩张对术后复发无明显统计学意义(P>0.05)。结论对于ERCP取石术后患者采用胆胃舒颗粒治疗,具有利胆通腑、清热化湿、凉血止血功效,可降低结石远期复发率。Objective To observe the long-term effect of Danweishu Granules in preventing the recurrence of choledocholithiasis after ERCP. Methods 193 patients undergoing ERCP choledocholithotomy were retrospectively analyzed,and the COX regression model was used to determine whether to give TCM prevention,gender,age,common bile duct diameter,stone size,number of stones,diverticulum,common bile duct 12 intestinal fistula,EST incision size,nipple balloon dilation,intraoperative lithotripsy and other 11 factors that might affect the recurrence of choledocholithiasis were analyzed. Results During 6 to10 years after ERCP,the cumulative recurrence probability of common bile duct stones in TCM prevention group and control group were 8.5% and 19.1%,respectively,and the difference was statistically significant(P<0.05);among them,whether the patient had TCM prevention,the number of stones,the diameter of the common bile duct,and intraoperative lithotripsy factors had significant statistical significance for postoperative recurrence(P<0.05);the patients’ age,gender,common bile duct stone size,diverticula,common bile duct itestinal fistula,EST small/medium incision,and nipple balloon dilation had no significant statistical significance for postoperative recurrence(P>0.05). Conclusion For patients after ERCP stone removal,Danweishu Granules have the effects of promoting gallbladder,clearing away heat and removing dampness,cooling blood and stopping bleeding,and can reduce the long-term recurrence rate of stones.
