TRPC6与糖尿病肾病足细胞损伤的相关机制研究及中医药治疗进展

摘　　要：糖尿病肾病(DN)是糖尿病的常见并发症,也是终末期肾病的主要病因之一。蛋白尿是DN最直接、最主要的表现,然而蛋白尿的出现与足细胞的损伤密切相关。DN的主要病理变化是肾小球基底膜增厚、肾小球细胞肥大和足细胞损伤及丢失。足细胞是肾小球滤过屏障的重要组成部分,是保障肾小球滤过功能的关键。足细胞功能的正常发挥严重依赖胞内钙信号的处理,过量的Ca2+内流会导致足细胞足突融合、消失及细胞的凋亡,进一步导致肾小球的损伤。TRPC6是介导足细胞钙内流最关键的蛋白,其表达水平与DN等蛋白尿性肾脏疾病的发生发展密切相关。西医对DN尚无直接有效的治疗方法,多以降低其相关危险因素为主。中医对DN的认识由来已久,以其治疗形式多样、成本低廉、疗效确切被广泛应用于DN的临床治疗。本文对TRPC6的表达与DN中足细胞损伤的相关机制及中医药对DN的干预作简要的总结和概括。Diabetic nephropathy(DN)is a common complication of diabetes and one of the main causes of end-stage renal disease(ESRD).Albuminuria is the most direct and main manifestation of DN.However,the occurrence of proteinuria is closely related to podocyte injury.The main pathological changes of DN are thickening of glomerular basement membrane,hypertrophy of glomerular cells and injury and loss of podocytes.Podocyte is an important part of glomerular filtration barrier and the key to ensure glomerular filtration function.The normal function of podocytes depends heavily on the processing of intracellular calcium signals.Excessive Ca^(2+)influx leads to podocyte foot process fusion,disappearance and cell apoptosis,and further leads to glomerular injury.TRPC6 is the most important protein mediating calcium influx into podocytes,and its expression level is closely related to the occurrence and development of proteinuric renal diseases such as DN.Western medicine has no direct and effective treatment for DN,mostly to reduce its related risk factors.TCM has known DN for a long time,and it has been widely used in the treatment of DN because of its various treatment forms,low cost and definite curative effect.This article briefly summarized the expression of TRPC6 and the related mechanism of podocyte injury in DN and the intervention of TCM on DN.
