三级甲等医院-社区联动助力社区伤口护理专业化发展

摘　　要：目的:实践以促进社区伤口护理专业化发展为目标的三级甲等医院-社区联动工作模式,分析其效果。方法:利用3C战略三角模型,制定三级甲等医院-社区联动策略,不断推进社区伤口服务专业化。结果:自2016年三级甲等医院-社区联动实施以来,社区每月伤口护理人数、人次分别由2016年的12.5人、36.5人次上升到2019年65.5人、185.5人次(P<0.05),上门换药次数从2016年的0次到2019年的130次,伤口种类也从2016年的6种增加到2019年的17种。由社区上转至三级甲等医院患者人次、由三级甲等医院下转至社区的患者人次及三级甲等医院伤口护理专家到社区会诊的次数在2016年均为0,到2019年分别增至56人次、20人次和48次。结论:以3C战略模型为依据制定的三级甲等医院-社区联动策略有效地促进了社区伤口护理专业化发展,激励和带动了社区伤口专科护理能力。Objective:To explore the tertiary grade A hospital-community hospital linkage model based on the promotion of wound care specialization in community,and analyze its effects.Methods:The 3C strategic triangle model was used to develop specific hospital-community linkage strategies,to promote wound care specialization continuously.Results:Since the implementation of 3A-hospital-community linkage in 2016,the median number of patients,person-time and income of community wound care increased from 12.5 and 36.5 in 2016 to 65.5 and 18.5 in 2019 respectively(P<0.05).The frequency of home-based dressing change increased from 0 times in 2016 to 130 times in 2019,and the number of wound types increased from 6 in 2016 to 17 in 2019.The number of patients transferred from community hospitals to 3A hospitals,the number of patients transferred from 3A hospitals to community hospitals,and the number of consultation by wound care experts in community hospitals increasing from 0 in 2016 to 56,20 and 48 respectively in 2019 and becoming stable gradually.Conclusion:The 3A-class-tertiary-hospital-community-hospital linkage strategy based on 3C strategic model has effectively promoted the development of community wound specialization,improved the wound care ability of community nurses.
