健脾益肾汤治疗脾肾气虚型小儿过敏性紫癜临床研究

摘　　要：目的:观察健脾益肾汤治疗脾肾气虚型小儿过敏性紫癜的临床疗效。方法:选取82例脾肾气虚型过敏性紫癜患儿,根据不同治疗方案分为观察组45例和对照组37例。对照组给予常规对症治疗,观察组在对照组基础上给予健脾益肾汤治疗,2组均治疗4周。比较2组临床疗效;比较2组治疗前后血清白细胞介素-9 (IL-9)、白细胞介素-17 (IL-17)、肿瘤坏死因子-α(TNF-α)、免疫球蛋白G (IgG)水平及尿微量白蛋白(mA)、尿α1-微球蛋白(α1-M)水平。结果:观察组临床疗效优于对照组(Z=3.135,P<0.05)。治疗后,观察组血清IL-9、IL-17、TNF-α、IgG水平及尿mA、尿α1-M水平均低于对照组(P<0.05)。结论:健脾益肾汤治疗脾肾气虚型小儿过敏性紫癜临床疗效较好,可显著降低患儿的血清炎症因子水平及尿mA、尿α1-M水平,改善肾功能。Objective:To observe the clinical effect of Jianpi Yishen tang for allergic purpura in children of spleen-kidney qi deficiency type. Methods:A total of 82 cases of children with allergic purpura of spleen-kidney qi deficiency type were selected and divided into the observation group and the control group according to their different treatment schemes,45 and37 cases in each group respectively. The control group was given routine symptomatic treatment,and the observation group was additionally treated with Jianpi Yishen tang based on the treatment of the control group. Both groups were treated for four weeks. The levels of interleukin-9(IL-9),interleukin-17(IL-17),tumor necrosis factor-α(TNF-α),immunoglobulin G(IgG) in serum and microalbumin(mA) and α1-microglobulin(α1-M) in urine before and after treatment,and the clinical effect in the two groups were compared. Results:The clinical effect in the observation group was better than that in the control group(Z=3.135,P<0.05). After treatment,the levels of IL-9,IL-17,TNF-α and IgG in serum,as well as mA and α1-M in urine in the observation group were lower than those in the control group(P<0.05). Conclusion:Jianpi Yishen tang for allergic purpura in children of spleen-kidney qi deficiency type has better clinical effect,which can effectively reduce the levels of inflammatory factors in serum and mA and α1-M in urine in children,and improve kidney function.
