摘 要:目的:观察清金逐瘀汤辅助西医治疗重症肺炎的临床疗效。方法:将110例重症肺炎患者按随机数字表法分为对照组和观察组各55例。对照组给予常规西医对症治疗,观察组在对照组基础上给予清金逐瘀汤治疗。比较2组临床疗效、治疗前后血清氧化应激因子、炎症因子、铜蓝蛋白(CER)、水通道蛋白1 (AQP1)水平。结果:观察组总有效率为89.09%,高于对照组74.55%,差异有统计学意义(P<0.05)。2组治疗前CER、AQP1、丙二醛(MDA)、超氧化物歧化酶(SOD)、白细胞介素-1 (IL-1)、白细胞介素-13 (IL-13)、肿瘤坏死因子-α(TNF-α)水平比较,差异无统计学意义(P>0.05)。治疗后,2组SOD水平高于治疗前,CER、AQP1、MDA、IL-1、IL-13、TNF-α水平低于治疗前(P<0.05);观察组SOD水平高于对照组,CER、AQP1、MDA、IL-1、IL-13、TNF-α水平低于对照组(P<0.05)。2组治疗前临床肺部感染(CPIS)评分、急性生理与慢性健康状况评分Ⅱ(APACHEⅡ)评分比较,差异无统计学意义(P>0.05)。治疗后,2组CPIS评分、APACHEⅡ评分较治疗前降低(P<0.05),观察组CPIS评分、APACHEⅡ评分低于对照组(P<0.05)。结论:清金逐瘀汤辅助西医治疗重症肺炎可减轻炎症反应和氧化应激损伤,调节CER、AQP1的表达水平,提高临床疗效。Objective: To observe the clinical effect of Qingjin Zhuyu tang assisting western medicine for severe pneumonia. Methods:A total of 110 cases of patients with severe pneumonia were divided into the control group and the observation group according to the random number table method, 55 cases in each group. The control group was given symptomatic treatment with routine western medicine,and the observation group was additionally given Qingjin Zhuyu tang based on the treatment of the control group. The levels of oxidative stress factors,inflammatory factors,Ceruloplasmin(CER)and Aquaporin 1(AQP1) in serum before and after treatment as well as the clinical effect in the two groups were compared.Results:The total effective rate was 89.09% in the observation group,higher than that of 74.55% in the control group,the difference being significant(P<0.05). Before treatment,there were no significant difference being found in the comparison of the levels of CER,AQP1,malondialdehyde(MDA),superoxide dismutase(SOD),interleukin-1(IL-1),interleukin-13(IL-13)and tumor necrosis factor-α(TNF-α) between the two groups(P>0.05). After treatment,SOD levels in the two groups were higher than those before treatment,and the levels of CER,AQP1,MDA,IL-1,IL-13 and TNF-α were lower(P<0.05);SOD level in the observation group was higher than that in the control group,and the levels of CER,AQP1,MDA,IL-1,IL-13 and TNF-α in the observation group were lower(P<0.05). Before treatment,there was no significant difference being found in the comparison of the scores of Clinical Pulmonary Infection Score(CPIS) and Acute Physiological and Chronic Health Evaluation Ⅱ(APACHEⅡ) between the two groups(P>0.05). After treatment,the scores of CPIS and APACHEⅡ in the two groups were decreased when compared with those before treatment(P<0.05), and the above scores in the observation group were lower than those in the control group(P<0.05).
