手法松解联合温通膏治疗冻结肩临床观察

摘　　要：目的:观察手法松解联合温通膏外敷治疗冻结肩的临床疗效。方法:将69例冻结肩患者随机分为3组,手法松解组、联合组、温通膏组。分别于治疗前、治疗1周、治疗2周、治疗4周后测定Constant-Murley肩关节评分[包括疼痛、日常生活能力(ADL)、关节活动度(ROM)、总分],比较治疗前后各项评分的变化。结果:治疗1周后,临床疗效总有效率手法组77.27%,温通膏组68.18%,联合组86.36%,3组间比较,差异无统计学意义(P>0.05)。治疗2周后,临床疗效总有效率手法组86.36%,温通膏组77.27%,联合组90.91%,3组间比较,差异无统计学意义(P>0.05)。治疗4周后,临床疗效总有效率手法组90.91%,温通膏组90.91%,联合组95.45%,3组间比较,差异有统计学意义(P<0.01)。Constant-Murley肩关节评分中,治疗4周后,联合组疼痛、ADL、ROM、总分等各项评分均明显优于手法组及温通膏组,差异均有统计学意义(P<0.05)。且手法组疼痛、ADL、ROM、总分等各项评分分别与温通膏组比较,差异均无统计学意义(P>0.05)。3组治疗患者均未发现不良反应。结论:手法松解联合温通膏外敷治疗冻结肩在缓解疼痛、增加活动度上具有良好的疗效;单纯温通膏外敷治疗在治疗1周、2周时疗效或相当或不及手法松解,但在治疗4周时疗效与手法松解相当。Objective: To observe the clinical effect of manipulation release combined with the external application of Wentong ointment for periarthritis humeroscapularis. Methods: A total of 69 cases of patients with periarthritis humeroscapularis were randomly divided into three groups,namely,the manipulation group,the combination group,and the Wentong ointment group. Before treatment as well as after one-week, two-week, and four-week treatment,Constant-Murley Score for evaluating the function of shoulder joint [pain,activities of daily living(ADL),range of motion(ROM),and the total score] was determined, and the changes in each score above before and after treatment were compared.Results: After one-week treatment, the total effective rate of clinical effect was 77.27% in the manipulation group,68.18% in the Wentong ointment group,and 86.36% in the combination group,no significance in the difference among the three groups(P>0.05). After two-week treatment,the total effective rate of clinical effect was 86.36% in the manipulation group, 77.27% in the Wentong ointment group, and 90.91% in the combination group, no significance in the difference among the three groups(P>0.05). After four-week treatment, the total effective rate of clinical effect was 90.91% in the manipulation group,90.91% in the Wentong ointment group,and 95.45% in the combination group,the difference being significant among the three groups(P<0.01). After four-week treatment,the scores of pain,ADL,ROM,as well as the total score of Constant-Murley Score in the combination group were significantly better than those in the manipulation group and the Wentong ointment group(P<0.05). After four-week treatment,when compared scores of pain,ADL,ROM,and total score between the manipulation group and the Wentong ointment group,there was no significance in differences(P>0.05).
