摘　　要：目的探讨疣状血管瘤的手术方式,评价手术治疗效果。方法 69例疣状血管瘤患儿,根据血管瘤位置、宽度、深度及病变影响关节活动的程度,分别采用血管瘤切除术、血管瘤切除术联合任意皮瓣成形术/皮肤移植术/带蒂皮瓣或穿支皮瓣成形术治疗。术后应用抗生素预防创面及植皮区感染,应用抗凝、扩血管药物预防皮瓣血管痉挛、栓塞,石膏托固定或辅助制具协助改善关节活动,并进行功能康复训练。随访6-12个月,结合彩超及MRI检查结果评估治愈、延迟愈合、复发情况。结果 57例行单次手术,11例行2期手术,1例行3期手术。随访至2020年11月,69例均治愈,其中5例延迟愈合。结论疣状血管瘤首选手术治疗,重症患者可联合多种手术方式,分期进行,近期疗效满意,复发率低。Objective To explore the surgical methods of verrucous hemangioma and evaluate the surgical outcomes.Methods Sixty-nine children with verrucous hemangioma received hemangioma resection,or hemangioma resection combined with any flap plasty,skin transplantation,pedicle flap or perforator flap plasty respectively according to the location,width and depth of the tumor.Antibiotics were used to prevent wound and skin graft infection after operation.Anticoagulant and vasodilator drugs were used to prevent vascular spasm and embolism of skin flap.Plaster brace fixation or rehabilitation devices were used to improve joint activity.Functional rehabilitation training was conducted.The patients were followed up for 6 to 12 months to evaluate the healing,recurrence and delayed healing with color ultrasound and MRI.Results Fifty-seven patients underwent single operation,11 underwent secondary operation,and 1 underwent three-stage operation.The patients were followed up till November,2020,and all patients were cured,in which 5 patients got delayed healing.Conclusion Operation is the first choice for verrucous hemangioma.The severe patients could adopt combination of multiple and staged surgery.The short-term efficacy is satisfactory and the recurrence rate is low.
