摘　　要：目的探讨血清淀粉样蛋白A(SAA)、C反应蛋白(CRP)和血常规中白细胞(WBC)计数等指标,在协助儿童乙型流感早期诊断中的临床价值。方法检测114例乙型流感组患儿和106例对照组儿童的SAA、CRP、血常规中白细胞(WBC)计数水平,计算SAA/CRP比值。分析两组间白细胞(WBC)计数、SAA、CRP、SAA/CRP比值的关系,并采用受试者工作特征曲线(ROC)评价各指标在判断乙型流感病毒感染方面的诊断效能。结果乙型流感组SAA、SAA/CRP、WBC计数与对照组相比,差异有显著统计学意义(P<0.01)。乙型流感组CRP与对照组相比,差异有统计学意义(P=0.01)。ROC曲线分析显示,乙型流感组SAA、SAA/CRP的曲线下面积(AUC)均>0.8(P<0.01),灵敏度分别为85.10%、73.70%,特异性分别为80.20%、92.50%。结论SAA、SAA/CRP比值可作为病毒感染的诊断指标,可以协助早期诊断儿童乙型流感病毒感染,对开展早期治疗具有指导意义,具有一定的临床价值。Objective To investigate the clinical value of serum amyloid A(SAA),C-reactive protein(CRP)and white blood cell(WBC)count in the early diagnosis of influenza B in children.Methods The levels of SAA,CRP and white blood cell(WBC)count in 114 children with confirmed influenza B and 106 non-infected children were detected,and the ratio of SAA/CRP was calculated.The relationship of SAA,CRP,SAA/CRP ratio and white blood cell(WBC)count between the two groups was analyzed,and the efficacy of diagnosis of influenza B virus infection was analyzed by receiver operating characteristic curve(ROC).Results There were significant differences in SAA,SAA/CRP and WBC count between the two groups(P<0.01).There was a significant difference in CRP level between the two groups(P=0.01).ROC curve analysis showed that AUC of SAA and SAA/CRP in children with influenza B were>0.8(P<0.01),with the sensitivity and specificity of 85.10%and 80.20%respectively.Conclusion SAA and SAA/CRP ratio can be applied as diagnostic indicators of viral infection.The combined detection of SAA,CRP and WBC count can facilitate the early diagnosis of influenza B in children,which has a guiding significance for the early treatment.
