摘 要:目的分析阻塞性睡眠呼吸暂停低通气综合征(OSAHS)患者血清转化生长因子β1(TGF-β1)、过氧化物酶体增殖物激活受体γ(PPAR-γ)、内皮素-1(ET-1)水平及其与肺动脉收缩压(PASP)的关系。方法选取2016年8月至2020年8月本院收治的85例OSAHS患者为研究对象,作为观察组,根据病情程度分为轻度组(n=28)、中度组(n=32)及重度组(n=25),另选取30例同期于我院进行体检的健康者作为对照组,检测血清TGF-β1、PPAR-γ、ET-1水平和PASP,采用ROC曲线预测血清TGF-β1、PPAR-γ、ET-1水平诊断OSAHS的价值,并分析血清TGF-β1、PPAR-γ、ET-1水平与PASP的相关性。结果观察组血清TGF-β1、PPAR-γ、ET-1水平及PASP均高于对照组(P<0.05);重度组血清TGF-β1、PPAR-γ、ET-1水平及PASP高于轻度组和中度组,中度组血清TGF-β1、PPAR-γ、ET-1水平及PASP高于轻度组(P<0.05);ROC曲线分析显示,血清TGF-β1、PPAR-γ、ET-1水平联合预测OSAHS的灵敏度、AUC高于单独检测(P<0.05);Spearman相关性分析显示,血清TGF-β1、PPAR-γ、ET-1水平与PASP呈正相关(P<0.05)。结论OSAHS患者血清TGF-β1、PPAR-γ、ET-1水平呈高表达,对OSAHS预测价值较高,且与PASP呈正相关性。Objective To analyze the relationship between serum transforming growth factorβ1(TGF-β1),peroxisome proliferation-activated receptorγ(PPAR-γ)and endothelin-1(ET-1)levels and pulmonary artery systolic pressure(PASP)in patients with obstructive sleep apnea-hypopnea syndrome(OSAHS).Methods 85 cases of OSAHS patients admitted to our hospital from August,2016 to August,2020 were selected as the research objects.They were divided into the mild group(n=28),moderate group(n=32)and severe group(n=25)according to the degree of illness.Another 30 cases of healthy people who underwent physical examination in our hospital during the same period were selected as the control group.Serum levels of TGF-β1,PPAR-γ,ET-1 and PASP were detected in all subjects.ROC curve was applied to predict the diagnostic value of serum levels of TGF-β1,PPAR-γ,and ET-1 in OSAHS,and the correlation between serum levels of TGF-β1,PPAR-γ,ET-1 and PASP was analyzed.Results The serum levels of TGF-β1,PPAR-γ,ET-1 and PASP in the observation group were higher than those in the control group(P<0.05).The serum levels of TGF-β1,PPAR-γ,ET-1 and PASP in the severe group were higher than those in the mild group and the moderate group,and the serum levels of TGF-β1,PPAR-γ,ET-1 and PASP in the moderate group were higher than those in the mild group(P<0.05).ROC curve analysis showed that the combined prediction of serum levels of TGF-β1,PPAR-γand ET-1 had a higher sensitivity and AUC value in predicting OSAHS,compared with single-index prediction(P<0.05).Spearman correlation analysis showed that serum levels of TGF-β1,PPAR-γand ET-1 were positively correlated with PASP(P<0.05).Conclusion The serum levels of TGF-β1,PPAR-γand ET-1 are highly expressed in OSAHS patients,which have a high predictive value for OSAHS and also a positive correlation with PASP.
