摘 要:目的探究妊娠期糖尿病(GDM)母亲的新生儿血清25-羟基维生素D_(3)[25(OH)D_(3)]、胰岛素水平变化及与胰岛素抵抗的相关性。方法选取2017年5月至2020年5月本院产科收治的GDM孕妇及其新生儿125例作为研究对象(GDM组),另选取同期在本院分娩的健康孕妇及其足月新生儿121例作为正常对照组。比较两组血清25(OH)D_(3)、胰岛素、血糖、HOMA-IR水平;采用Pearson法分析出生24h时GDM母亲的新生儿血清25(OH)D_(3)与胰岛素水平两者间的相关性及分别与血糖、HOMA-IR的相关性。结果出生后6、24h及48h时GDM组新生儿血清25(OH)D_(3)、血糖水平均明显低于正常对照组,血清胰岛素、HOMA-IR水平均明显高于正常对照组(P<0.05)。GDM组新生儿出生后6、24h及48h时血清25(OH)D_(3)、血糖水平依次逐渐显著升高,胰岛素、HOMA-IR水平依次逐渐显著降低(P<0.05)。出生24h时GDM组新生儿血清25(OH)D_(3)与胰岛素水平呈负相关(r=-0.504,P<0.001)。出生24h时GDM组新生儿血清25(OH)D_(3)与血糖水平呈正相关(r=0.447,P<0.001),与HOMA-IR水平呈负相关(r=-0.609,P<0.001);血清胰岛素与血糖水平呈负相关(r=-0.501,P<0.001),与HOMA-IR水平呈正相关(r=0.549,P<0.001)。结论GDM母亲的新生儿血清25(OH)D_(3)水平明显降低,血清胰岛素水平明显升高,检测两者水平可能对临床监控新生儿健康状况有一定价值。Objective To explore the changes of serum 25 hydroxyvitamin D3(25(OH)D_(3))and insulin levels in newborns of gestational diabetes mellitus(GDM)mothers and their correlation with insulin resistance.Methods From May,2017 to May,2020,125 pregnant women with GDM and their newborns in the Obstetrics Department of our hospital were selected as the research objects(GDM group),while 121 healthy pregnant women and their full-term newborns delivered in our hospital during the same period were selected as the normal control group.The levels of serum 25(OH)D_(3),insulin,blood glucose and HOMA-IR were compared between the two groups.Pearson method was used to analyze the correlation between serum 25(OH)D_(3)and insulin levels in newborns with GDM mothers at 24 hours of birth,as well as the correlation with blood glucose and HOMA-IR,respectively.Results At 6 h,24 h and 48 h after birth,the neonatal serum 25(OH)D_(3)and blood glucose levels in the GDM group were significantly lower than those in the normal control group,while the serum insulin and HOMA-IR levels were significantly higher than those in the normal control group(P<0.05).The serum 25(OH)D_(3)and blood glucose levels of the newborns in the GDM group were significantly increased at 6,24,and 48 h after birth,while the insulin and HOMA-IR levels were significantly decreased sequentially(P<0.05).There was a negative correlation between serum 25(OH)D_(3)and insulin levels of newborns in the GDM group at 24 hours of birth(r=-0.504,P<0.001).Serum 25(OH)D_(3)was positively correlated with blood glucose level of newborns in the GDM group(r=0.447,P<0.001),while negatively correlated with HOMA-IR level(r=-0.609,P<0.001)at 24 h after birth.Serum insulin was negatively correlated with blood glucose(r=-0.501,P<0.001),while positively correlated with HOMA-IR(r=0.549,P<0.001).Conclusion The level of serum 25(OH)D_(3)in infants of GDM mothers is significantly lower,and the level of serum insulin is significantly higher.The detection of levels of these two markers may have a certain
