泻火解毒方医治蝮蛇咬伤的效果观察

摘　　要：目的观察泻火解毒方医治蝮蛇咬伤的效果。方法收集安徽中医药大学第一附属医院急诊科住院治疗的53例蝮蛇咬伤患者,其中泻火解毒方治疗组为27例,对照组为26例。比较分析所有患者治疗前后的相关血液指标,包含白细胞计数(white blood cell,WBC)、中性粒细胞计数(neutrophil granulocyte,NEUT)、血小板计数(blood platelet,PLT)、丙氨酸氨基转移酶(alanine aminotransferase,ALT)、天门冬氨酸氨基转移酶(aspartate transaminase,AST)、肌酸激酶(creatine kinase,CK)、肌酸激酶同工酶(creatine kinase-MB,CK-MB)的变化情况;对比2组患者治疗前后肢体疼痛以及肿胀消散的时间;对比2组患者平均住院天数以及总的治疗费用。结果两组患者在治疗前,相关血液检查水平差别均不大(P>0.05)。使用泻火解毒方治疗组治疗后WBC、NEUT、ALT、AST、CK、CK-MB的下降水平以及PLT的上升水平均好于对照组,且两组之间的差异有统计学意义(P<0.05)。与对照组对比,泻火解毒方治疗组治疗后患者肢体疼痛、肿胀的消失时间均明显优于对照组(P<0.05)。泻火解毒方治疗组治疗后其平均住院时间明显短于对照组,差异有统计学意义(P<0.05),两组间患者住院总费用差异无统计学意义(P>0.05)。结论对治疗蝮蛇咬伤患者,联合泻火解毒方内服的综合诊疗方法可以有效医治蝮蛇咬伤。Objective To observe the effect of Xiehuo Jiedu Decoction in treating Agkistrodon halys bite wounds.Methods 53 patients with Agkistrodon halys bite wounds in the Emergency Department of the First Affiliated Hospital of Anhui University of Traditional Chinese Medicine were collected for the study,27 cases in the Xiehuo Jiedu Decoction treatment group and 26 cases in the control group.The blood indexes of all patients before and after the treatment were compared and analyzed,including white blood cell(WBC),neutrophil granulocyte(neut),blood platelet(PLT),alanine aminotransferase(ALT),and aspartate aminotransferase(AST).The changes of serum creatine kinase(CK),creatine kinase-MB(CK-MB)were observed.We compared the length of pain and swelling dissipation of the limbs before and after the treatment in the two groups.We also compared the average number of days in hospital and the total treatment cost of the two groups.Results Before the treatment,the blood examination level of the two groups was not different(P>0.05).The decrease of WBC,NEUT,ALT,AST,CK,CK-MB and the increase of PLT in the treatment group were better than those in the control group,and the difference between the two groups was statistically significant(P<0.05).Compared with the control group,the disappearance time of limb pain and swelling in Xiefao Jiedu treatment group were significantly better than those of the control group(P<0.05).After the treatment,the average hospitalization time of Xiehuo Jiedu Decoction treatment group was significantly shorter than that of the control group(P<0.05),and there was no significant difference in the total hospitalization expenses between the two groups(P>0.05).Conclusion In the treatment of patients with Agkistrodon halys bite wounds,combined with Xiehuo Jiedu Decoction can effectively improve the treatment outcomes.
