摘 要:目的探讨性早熟女童血清性激素、胰岛素样生长因子-Ⅰ(IGF-Ⅰ)、瘦素(Leptin)水平与生长发育的关系。方法选取2019年1月至2019年12月性早熟女童155例,分为中枢性性早熟117例(CPP组)和外周性性早熟38例(PPP组)。选取40例年龄配对的无第二性征发育的健康女童为对照组。CPP组、PPP组在治疗前,对照组在体检时,分别检测血清促黄体生成素(LH)、促卵泡激素(FSH)、IGF-Ⅰ、Leptin水平以及身高、体重、骨龄、卵巢容积。CPP组在药物治疗3个月、6个月、1年后再次检测上述指标。结果在未进行任何治疗前,CPP组的血清LH、FSH、IGF-Ⅰ、Leptin水平以及身高、体重、骨龄、卵巢容积均大于PPP组和对照组(P<0.05)。血清LH、FSH水平与卵巢容积呈正相关(r=0.624,r=0.553,P<0.05)。血清IGF-Ⅰ、Leptin水平与骨龄呈正相关(r=0.493,r=0.408,P<0.05)。血清LH、FSH、IGF-Ⅰ、Leptin水平与身高、体重无相关性(P>0.05)。治疗3个月、6个月、1年后,CPP组血清LH、FSH、IGF-Ⅰ、Leptin水平均下降(P<0.05),身高、体重、骨龄呈缓慢增长趋势,年均增长幅度小于对照组(P<0.05)。结论性早熟女童血清性激素、IGF-Ⅰ、Leptin水平与生长发育密切相关,其水平升高可促进骨龄增长、卵巢增大;药物治疗可使血清性激素、IGF-Ⅰ、Leptin水平下降,调整生长发育速度。Objective To investigate the relationship between levels of serum sex hormones, insulin-like growth factor-Ⅰ(IGF-Ⅰ), leptin(Leptin) and the development of girls with precocious puberty.Methods 155 girls with precocious puberty admitted to the hospital between January and December 2019 were selected and divided into the central precocious puberty(CPP group, 117 cases) and peripheral precocious puberty(PPP group, 38 cases) groups. Meanwhile, 40 age-matched healthy girls without secondary sexual development were recruited as the control group. The levels of serum luteinizing hormone(LH), follicle stimulating hormone(FSH), IGF-Ⅰ and leptin, as well as height, weight, bone age, and ovarian volume of the three groups were measured. The above indexes in the CPP group were tested again after 3 months, 6 months and 1 year of drug treatment.Results Before treatment, the levels of serum LH, FSH, IGF-Ⅰ and leptin, also values of height, weight, bone age, and ovarian volume of the CPP group were greater than those of the PPP group and the control group(P<0.05). Serum LH and FSH levels were positively correlated with ovarian volume(r=0.624, r=0.553, P<0.05). Serum IGF-Ⅰ and leptin levels were positively correlated with bone age(r=0.493, r=0.408, P<0.05). Serum LH, FSH, IGF-Ⅰ and leptin levels were not correlated with height and weight(P>0.05). After 3 months, 6 months, and 1 year of treatment, serum LH, FSH, IGF-Ⅰ, and leptin levels in the CPP group decreased(P<0.05), while values of height, weight, and bone age showed a slow increasing trend, and the average annual increase was smaller than that in the control group(P<0.05).Conclusion Serum sex hormones, IGF-Ⅰ and leptin levels in girls with precocious puberty are closely related to their development. Increases of these markers can promote increases in both bone age and ovarian volume. Drug treatment can lower the levels of serum sex hormones, IGF-Ⅰ and leptin, and adjust growth rate.
