透明质酸钠联合运动疗法治疗早期髌股关节炎的临床效果观察

摘　　要：目的观察关节腔注射透明质酸钠联合膝关节运动疗法治疗早期髌股关节炎的临床效果。方法135例早期髌股关节炎患者,根据随机数字表法分为联合组、透明质酸钠组及常规组,每组45例。联合组给予透明质酸钠联合运动疗法,透明质酸钠组单纯给予透明质酸钠治疗,常规组给予非甾体类消炎药治疗。比较三组患者治疗效果,治疗前后视觉模拟评分法(VAS)评分,Lysholm膝关节评分量表(LKSS)评分。结果治疗后1周,联合组、透明质酸钠组和常规组的治疗总有效率分别为91.11%、95.56%、91.11%,组间比较差异无统计学意义(P>0.05);治疗后5周,联合组、透明质酸钠组和常规组的治疗总有效率分别为95.56%、93.33%、71.11%,联合组、透明质酸钠组的治疗总有效率均优于常规组,差异具有统计学意义(P<0.05);治疗后12周,联合组、透明质酸钠组和常规组的治疗总有效率分别为97.78%、82.22%、35.56%,联合组治疗总有效率优于透明质酸钠组和常规组,透明质酸钠组治疗总有效率优于常规组,差异具有统计学意义(P<0.05)。治疗后1周,三组的VAS评分均低于本组治疗前,差异具有统计学意义(P<0.05),但三组VAS评分比较差异无统计学意义(P>0.05);治疗后5周,三组的VAS评分均低于本组治疗前,且联合组及透明质酸钠组均低于常规组,差异具有统计学意义(P<0.05);治疗后12周,三组的VAS评分均低于本组治疗前,且联合组的VAS评分低于透明质酸钠组和常规组以及本组治疗后1周和5周,透明质酸钠组低于常规组,差异具有统计学意义(P<0.05)。治疗后1周,三组的LKSS评分均高于本组治疗前,差异具有统计学意义(P<0.05);治疗后5周,三组的LKSS评分均高于本组治疗前,联合组LKSS评分高于透明质酸钠组及常规组,透明质酸钠组LKSS评分高于常规组,差异具有统计学意义(P<0.05)。治疗后12周,三组的LKSS评分均高于本组治疗前,联合组LKSS评分高于透�Objective To observe the clinical effect of sodium hyaluronate(SH)combined with exercise therapy in the treatment of early patellofemoral arthritis.Methods A total of 135 patients with early patellofemoral arthritis were divided into combination group,sodium hyaluronate group and conventional group according to random numerical table,with 45 cases in each group.The combination group was treated with sodium hyaluronate and exercise therapy,sodium hyaluronate group was treated with sodium hyaluronate only,and conventional group was treated with non-steroidal anti-inflammatory drugs.The therapeutic effect,visual analogue scale(VAS)score,Lysholm knee score scale(LKSS)score of the three groups were compared.Results 1 week after treatment,the total effective rate of the combination group,sodium hyaluronate group and conventional group were 91.11%,95.56%and 91.11%respectively,and the difference was not statistically significant(P>0.05).5 weeks after treatment,the total effective rate of the combination group,sodium hyaluronate group and conventional group were 95.56%,93.33%and 71.11%;the total effective rate of the combination group and sodium hyaluronate group were better than that of the conventional group,and the difference was statistically significant(P<0.05).12 weeks after treatment,the total effective rate of the combination group,sodium hyaluronate group and conventional group were 97.78%,82.22%and 35.56%;the total effective rate of the combination group was better than that of the sodium hyaluronate group and conventional group,and sodium hyaluronate group was better than the conventional group,and the difference was statistically significant(P<0.05).1 week after treatment,the VAS score of the three groups were lower than those before treatment,and the difference was statistically significant(P<0.05),but there was no statistically significant difference in VAS score among the three groups(P>0.05).5 weeks after treatment,the VAS score of the three groups were lower than those before treatment,and the combination
