摘  要:目的探讨胱抑素C(CysC)和尿素(Urea)、同型半胱氨酸(Hcy)联合检测在2型糖尿病肾损伤早期诊断中的临床应用。方法185例2型糖尿病患者,按尿蛋白排泄率(UAER)不同分为单纯2型糖尿病组(75例)、早期糖尿病肾病组(59例)及临床期糖尿病肾病组(51例);另选取本院门诊健康体检者60例作为对照组。取所有受试者清晨空腹静脉血,分离血清待检,采用免疫比浊法检测CysC、Hcy,采用脲酶法检测Urea,采用氧化酶法检测肌酐(Cr)、尿酸(UA)。比较四组受试者的CysC、Hcy、Urea、Cr、UA,并计算CysC在早期糖尿病肾病组中的阳性率。结果单纯2型糖尿病组、早期糖尿病肾病组与对照组的Cr水平比较,差异无统计学意义(P>0.05);单纯2型糖尿病组及早期糖尿病肾病组的CysC、Urea、Hcy和UA均高于对照组,差异有统计学意义(P<0.05);早期糖尿病肾病组的CysC、Hcy高于单纯2型糖尿病组,差异有统计学意义(P<0.05);临床期糖尿病肾病组的CysC、Urea、Hcy、UA及Cr均高于对照组、单纯2型糖尿病组及早期糖尿病肾病组,差异有统计学意义(P<0.05)。早期糖尿病肾病组中有35例CysC阳性,CysC阳性率为59.32%(35/59)。结论CysC和Urea、Hcy联合检测可以反映2型糖尿病早期的肾损伤情况,可作为2型糖尿病肾损伤早期的敏感指标,对早期诊断和治疗具有重要意义。Objective To discuss the clinical application of combined test of cystatin C(Cys C),Urea and homocysteine(Hcy)in early diagnosis of renal injury in type 2 diabetes mellitus.Methods A total of 185 patients with type 2 diabetes mellitus were divided into simple diabetes group(75 cases),early diabetic nephropathy group(59 cases)and clinical diabetic nephropathy group(51 cases)according to urinary albamin excretion rate(UAER).Another 60 healthy subjects in outpatient department were selected as the control group.The fasting venous blood of all subjects was collected in the morning,and the serum was separated for testing.The immunoturbidimetric method was used to detect Cys C and Hcy,the urease method was used to detect Urea,and the oxidase method was used to detect creatinine(Cr)and uric acid(UA).The Cys C,Hcy,Urea,Cr and UA of the four groups were compared,and the positive rate of Cys C in the early diabetic nephropathy group was calculated.Results There was no statistically significant difference in Cr levels between the simple type 2 diabetes group,the early diabetic nephropathy group and the control group(P>0.05).The Cys C,Urea,Hcy and UA of simple type 2 diabetes group and early diabetic nephropathy group were higher than those of the control group,and the difference was statistically significant(P<0.05).The Cys C and Hcy of early diabetic nephropathy group was higher than those of simple type 2 diabetes group,and the difference was statistically significant(P<0.05).The Cys C,Urea,Hcy,UA and Cr of clinical diabetic nephropathy group were higher than those of the control group,simple type 2 diabetes group and early diabetic nephropathy group,and the difference was statistically significant(P<0.05).In the early diabetic nephropathy group,35 cases were Cys C positive,and the Cys C positive rate was 59.32%(35/59).Conclusion Combined test of Cys C,Urea and Hcy can reflect renal injury in early stage of type 2 diabetes mellitus,and can be used as a sensitive indicator of early renal injury of type 2 diabetes mellitus,
