刺血疗法治疗痛风性关节炎临床取穴规律的数据挖掘研究

摘　　要：目的:探索研究刺血疗法治疗痛风性关节炎的临床取穴规律分析。方法:收集近30年来刺血疗法治疗痛风性关节炎的临床文献,通过关键词“痛风性关节炎”“刺血”“刺络”“放血”对中国知网、万方、维普等数据库进行检索。采用EXCEL将所纳入文献的针灸处方进行归纳总结,建立数据库并做频数分析,分别采用SPSS19.00、SPSS modeler17进行聚类分析和关联规则分析。结果:符合并纳入44篇文献,穴位频次前10位由高到低依次为阿是穴、太冲、行间、内庭、三阴交、陷谷、太白、血海、阴陵泉、委中;归经频次前5位依次是脾经、肝经、胃经、胆经、膀胱经;特定穴频次前5位依次是输穴、合穴、荥穴、阿是穴、郄穴。聚类分析结果提示有5个有效聚类群:丘墟-曲池-委中-阳陵泉、血海-阴陵泉-太白、行间-内庭-三阴交-陷谷、太冲穴、阿是穴。关联规则分析结果显示,置信度与支持度最高为:行间-内庭-太冲。结论:临床上采用刺血疗法治疗痛风性关节炎,取穴规律以局部痛点和五腧穴为主。Objective:To explore and study the regularity of clinical acupoints selection in the treatment of gouty arthritis with pricking blood therapy.Methods:The clinical literature about the treatment of gouty arthritis with blood pricking therapy in the past 30 years was collected,and the databases of CNKI,Wanfang and VIP were searched with the keywords of‘gouty arthritis’,‘pricking blood’,‘pricking collateral’and‘bloodletting’.EXCEL was used to summarize the acupuncture prescriptions included in the literature in order to establish the database and do frequency analysis,also SPSS19 and SPSS modeler17 were applied for cluster analysis and association rule analysis.Results:44 articles were included in the study,it found that the top 10 acupoints in descending order of frequency were Ashi points,Taichong(LR3),Xingjian(LR2),Neiting(ST44),Sanyinjiao(SP6),Xiangu(ST43),Taibai(SP3),Xuehai(SP10),Yinlingquan(SP9)and Weizhong(BL40).The top 5 meridians with high frequency were the Spleen Meridian of Foot Taiyin,the Liver Meridian of Foot Jueyin,the Stomach Meridian of Foot Yangming,the Gallbladder Meridian of Foot Shaoyang and the Bladder Meridian of Foot Taiyang.The top 5 specific acupoints with high frequency were Shu-stream points,He-sea points,Xing-spring points,Ashi points and Xi-cleft points.The cluster analysis results suggested that there were 3 effective clusters,which were Qiuxu(GB40)-Quchi(LI11)-BL40-Yanglingquan(GB34),SP10-SP9-SP3,LR2-ST44-SP6-ST43,LR3 and Ashi points.The results of the association rule analysis showed that the cluster with highest degree of confidence and support was LR2-ST44-LR3.Conclusion:Clinically,pricking blood therapy is used to treat gouty arthritis,and the rule of the acupoints selected are mainly local tender points and five-Shu acupoints.
