经阴道超声在诊断子宫下段瘢痕妊娠中的价值

摘　　要：目的总结子宫下段瘢痕妊娠诊断方法,评价经阴道超声诊断价值,为子宫下段瘢痕妊娠诊断工作提供参考。方法选择收治的子宫下段瘢痕妊娠患者,总计50例。经阴道超声诊断,与手术病理诊断结果比较,观察经阴道超声诊断、手术病理诊断结果符合率情况。结果经阴道超声诊断检查确诊患者48例(27例患者子宫下段孕囊声像、周围血供来源瘢痕位置,14例患者子宫下段混合型包块且血流丰富,7例患者子宫下段包块回声、见丰富血流),误诊、漏诊等情况患者2例,与手术病理确诊子宫下段瘢痕妊娠情况差异无统计学意义,P>0.05。结论经阴道超声诊断子宫下段瘢痕妊娠准确率较高,能够明确患者的妊娠囊、子宫瘢痕关系情况,为患者临床治疗工作的开展提供参考。Objective To summarize the diagnostic methods of scar pregnancy in the lower uterine segment and evaluate the value of transvaginal ultrasound in the diagnosis of scar pregnancy in the lower uterine segment.Methods 50 cases of scar pregnancy in the lower segment of uterus were selected.The results of transvaginal ultrasound were compared with those of surgical pathology.Results 48 cases were diagnosed by transvaginal ultrasound(27 cases were diagnosed by ultrasonography of the lower uterine gestational sac,the location of scar around the blood supply source;14 cases were diagnosed by mixed mass of the lower uterine segment with rich blood flow;7 cases were diagnosed by echo of the lower uterine segment with rich blood flow);2 cases were diagnosed by misdiagnosis and missed diagnosis.There was no difference between the diagnosis and the diagnosis by operation and pathology,P>0.05.Conclusion Transvaginal ultrasound has a high accuracy in the diagnosis of scar pregnancy in the lower segment of the uterus,which can clarify the relationship between the gestational sac and scar of the uterus,and provide a reference for the clinical treatment of the patients.
