内眦动脉岛状鼻唇沟皮瓣修复鼻尖缺损15例观察

摘　　要：目的探讨内眦动脉岛状鼻唇沟皮瓣修复外伤导致鼻尖缺损的远期疗效。方法选取2010年1月至2018年4月河南省人民医院收治的采用内眦动脉岛状鼻唇沟皮瓣修复外伤鼻尖缺损15例,均为男性,年龄21~56岁,平均38岁;缺损范围包括鼻尖、部分鼻小柱和鼻翼,深度达软骨或软骨膜,缺损组织不能找到或无再植条件。人咬伤者换药5~7 d后手术,平均5.8 d,车祸伤者受伤至手术时间为1~6 h,平均3.4 h。手术全部成功,通过门诊随诊和家访随诊方式随访1~8年。结果本组得到随访的15例,随访时间1~8年,再造鼻尖外形良好,与周围组织肤色相近,质地一致,弹性佳。供瓣区无变形,瘢痕不明显。结论内眦动脉岛状鼻唇沟皮瓣一期修复外伤鼻尖缺损远期效果好,是临床上治疗的一种有效方法。Objective To investigate the long-term effect of Nasolabial fold island flap pedicled with Angular artery in repairing nasal tip defect caused by trauma.Methods From January 2010 to April 2018,15 cases with traumatic nasal tip defect were treated through Nasolabial fold island flap pedicled with Angular artery.All of the cases were male,The age ranged from 21 to 56 years old,with an average of 38 years old.The scope of the defect includes nasal tip,part of nasal columella and alar.and reaches the depth of cartilage or perichondrium.The defect tissues could not be found or replanted.Among the injured for human bite,the operation was performed 5 to 7 d(average:5.8 d)after dressing change,while among those for car accident,the operation was performed 1 to 6 h(average:3.4 h)after injured.All the operations were successful,Postoperative follow-up was from 1 to 8 years through outpatient service or home visit.Results The reconstructed nose tip was good in appearance and elasticity,and similar to the surrounding skin in color and texture.There was no obvious scar or deformation at the donor site.Conclusion The long-term effect of one-stage Nasolabial fold island flap pedicled with Angular artery repair on traumatic nasal tip defect is good,and is an effective method for clinical treatment.
