摘  要:目的观察分析基质金属蛋白酶-1(MMP-1)和环氧合酶-2(COX-2)的表达与瘢痕子宫试产结局的相关性。方法用免疫组化法检测瘢痕子宫先兆子宫破裂剖宫产组(观察组)和重复剖宫产组(对照组)产妇各32例,采集子宫、胎膜和胎盘组织,免疫组化积分比较其中MMP-1和COX-2的分布和表达,采用Logistic回归分析评估MMP-1和COX-2对试产结局的影响。结果观察组和对照组子宫、胎膜和胎盘细胞中均可见MMP-1和COX-2表达。不同组织中MMP-1和COX-2的表达无明显差异(P>0.05)。MMP-1免疫组化积分观察组子宫细胞(3.73±1.47),胎膜细胞(5.25±1.95),胎盘细胞(4.05±0.89);对照组子宫细胞(2.38±1.69),胎膜细胞(3.83±1.53),胎盘细胞(2.15±1.66)。观察组子宫、胎膜和胎盘细胞MMP-1表达水平显著高于对照组(P<0.05)。COX-2免疫组化积分观察组子宫细胞(3.40±1.43),胎膜细胞(4.22±1.54),胎盘细胞(3.91±1.06);对照组子宫细胞(1.88±1.56),胎膜细胞(2.27±1.07),胎盘细胞(2.02±0.97)。观察组子宫、胎膜和胎盘细胞COX-2表达水平显著高于对照组(P<0.05)。Logistic回归结果显示子宫、胎膜和胎盘中的MMP-1和COX-2对再次试产先兆子宫破裂的影响较为显著(P<0.05)。结论MMP-1,COX-2可能与分娩发动和宫颈成熟扩张等过程密切相关,在先兆子宫破裂发病机理中起到重要作用,对瘢痕子宫试产结局的实验室指标研究有一定的指导意义。Objective To observe and analyze the correlation between the expression of matrix metalloproteinase-1(MMP-1)and cyclooxygenase-2(COX-2)and the outcome of scar uterus.Methods Immunohistochemical method was used to detect 32 cases of cesarean section group(observation group)and 32 cases of repeated cesarean section group(control group)with scar uterus threatened uterine rupture.The uterus,fetal membrane and placenta tissues were collected to detect the immunohistochemical score,and the distribution and expression of MMP-1 and COX-2 were compared.Logistic regression was used to evaluate the effect of MMP-1 and COX-2 on the outcome of trial delivery.Results The expressions of MMP-1 and COX-2 were found in the uterus,fetal membrane and placenta of the observation group and the control group.There was no significant difference in the expression of MMP-1 and COX-2 in different tissues(P>0.05).In the observation group,the uterine cells were(3.73±1.47),the fetal membrane cells were(5.25±1.95),and the placental cells were(4.05±0.89);In the control group,the uterine cells were(2.38±1.69),the fetal membrane cells were(3.83±1.53),and the placental cells were(2.15±1.66).The expression level of MMP-1 in uterus,fetal membrane and placenta of observation group was significantly higher than that of control group(P<0.05).In the COX-2 immunohistochemical score observation group,uterine cells were(3.40±1.43),fetal membrane cells were(4.22±1.54),placental cells were(3.91±1.06);In the control group,uterine cells were(1.88±1.56),fetal membrane cells were(2.27±1.07),placental cells were(2.02±0.97).The expression level of COX-2 in uterus,fetal membrane and placenta of observation group was significantly higher than that of control group(P<0.05).The results of logistic regression showed that MMP-1 and COX-2 in uterus,fetal membrane and placenta had significant effect on threatened uterine rupture(P<0.05).Conclusion MMP-1 and COX-2 may play an important role in the pathogenesis of threatened uterine rupture,which may be closely
