超声造影对颅内动脉狭窄患者的诊断价值

摘  要:目的探讨超声造影(CEUS)对颅内动脉狭窄的诊断价值。方法选取40例单侧前循环动脉重度狭窄的颅内动脉狭窄患者。健康对照组20例。进行常规超声及超声造影的检查。计算达峰时间(PT)、脑循环时间(CCT)、脑血流量(CBF)、脑血容量(CBV)。比较两组间及狭窄组双侧超声指标的差别。结果狭窄组狭窄侧PT、CCT高于健侧及对照组(P<0.001);狭窄组狭窄侧CBF、CBV低于健侧及对照组(P<0.001)。结论CEUS是一种简单、经济、无创的方法;超声测量的PT、CCT延长、CBF、CBV降低对颅内动脉狭窄可能具有一定的提示作用。Objective To investigate the diagnostic value of contrast-enhanced ultrasonography(CEUS)in intracranial atherosclerotic stenosis.Methods Totally 40 consecutives intracranial atherosclerotic stenosis patients who had severe stenosis in unilateral anterior circulation were selected as stenosis group.20 consecutives healthy subjects were included as controls.They all underwent carotid ultrasonic examination.Then calculated peak time(PT)、cerebral circulation time(CCT)、cerebral blood flow(CBF)and cerebral blood volume(CBV).Compared the difference of the parameters between the two groups and between the bilateral sides of stenosis group.Results Compared to the normal side and controls,PT、CCT in the stenostic side of stenosis group were prolonged(P<0.001);CBF、CBV in stenostic side of stenosis group were lower than the normal side and controls(P<0.001).Conclusion CEUS is a simple,cheap and non-invasive examination.Prolonged PT、CCT and lowered CBF、CBV may indicate that there is a certain degree of intracranial arterial stenosis in patients.
