摘 要:目的探讨血清胶质细胞源性神经营养因子(GDNF)及脑源性神经营养因子(BDNF)水平监测在精神分裂症患者疗效评估中的临床价值。方法选取新乡医学院第二附属医院53例精神分裂症患者(2016年10月—2018年6月期间收治)设为观察组,接受为期8周的抗精神病药治疗,根据治疗效果分为(无效组、有效组、显效组、临床治愈组)4个亚组;同期选取51例健康体检者设为对照组,入组对象均抽血检测血清GDNF及BDNF水平。对比精神分裂症患者与健康者血清GDNF及BDNF水平、患病者治疗前后GDNF及BDNF水平变化、不同疗效者学血清GDNF及BDNF水平。结果观察组血清GDNF、BDNF水平低于对照组(P<0.05);观察组治疗后血清GDNF、BDNF水平高于治疗前(P<0.05);随疗效增加血清GDNF、BDNF水平亦有上升,两两比较差异有统计学意义(P<0.05)。结论精神分裂症患者血清GDNF、BDNF水平较健康者明显降低,但经抗精神病药物治疗可显著上调上述神经营养因子表达,且治疗后疗效越显著,其血清水平上升越高,在疗效评估中具有较高应用价值,其水平监测利于指导临床治疗方案调整。Objective To investigate the clinical value of serum glial cell line-derived neurotrophic factor(GDNF)and brain-derived neurotrophic factor(BDNF)levels in the evaluation of curative effect in patients with schizophrenia.Methods A total of 53 patients with schizophrenia(from October 2016 to June 2018)were enrolled in the observation group.They were treated with antipsychotic treatment for 8 weeks.According to the treatment effect,they were divided into two groups(ineffective group,effective group)There were 4 subgroups in the marked group and the clinical cure group.51 healthy subjects were selected as the control group at the same time.The blood samples were taken to detect serum GDNF and BDNF levels.Serum GDNF and BDNF levels in patients with schizophrenia and healthy subjects,changes in GDNF and BDNF levels before and after treatment,and serum GDNF and BDNF levels in patients with different curative effects.Results The levels of serum GDNF and BDNF in the observation group were lower than those in the control group(P<0.05).The levels of serum GDNF and BDNF in the observation group were higher than those before treatment(P<0.05).The serum levels of GDNF and BDNF also increased with the increase of curative effect.The difference between the two groups was statistically significant(P<0.05).Conclusion The levels of serum GDNF and BDNF in patients with schizophrenia are significantly lower than those in healthy subjects.However,the anti-psychotic drugs can significantly up-regulate the expression of these neurotrophic factors,and the therapeutic effect is more significant.The higher the serum level,the better in the evaluation of curative effect.It has high application value.Its level of monitoring is conducive to guiding the adjustment of clinical treatment options.
