环指指浅屈肌腱移位修复腱鞘炎小针刀松解术后拇长屈肌腱断裂的疗效观察

摘　　要：目的:探讨环指指浅屈肌腱移位修复腱鞘炎小针刀松解术后拇长屈肌腱断裂的临床疗效。方法:我们于2010年5月-2018年5月共收治16例腱鞘炎小针刀松解术后栂长屈肌腱断裂患者,应用环指指浅屈肌腱移位法,修复断裂的栂长屈肌腱。术后随访采用国际手外科肌腱疗效评估法(TAM法)与CrawfoM功能评定法评估患者术后拇指功能。结果:患者腱鞘炎小针刀松解术后至环指指浅屈肌腱移位的间隔时间为(15±3.8)天,术后患者随访时间为11-20个月,随访期间均无伤口感染、关节僵硬、肌腱黏连、手指畸形、肌腱再断裂等相关并发症发生。按照TAM法进行评估,其中优11例,良4例,可1例,差0例,优良率达93.8%;按照Crawford法进行肌腱功能评估,其中优9例,良5例,可2例,差0例,优良率达87.5%。结论:应用环指指浅屈肌腱移位,修复腱鞘炎小针刀松解术后拇长屈肌腱断裂可获得良好的临床效果,该手术操作简单,学习曲线平直,可以临床推广。Objective:To discuss the repair efficacy of flexor pollicis tendon rupture caused by small knife needle associated with tenosynovitis stenosans with transferring superficial flexor tendon of the ring finger.Methods:16 cases with flexor pollicis tendon rupture caused by small knife needle tendon were admitted to our hospital from May 2010 to May 2018,and repaired by transferring superficial flexor tendon of the ring finger.TAM score and Crawford score were used to assess the function of the affected finger after surgery.Re-sults:The interval between the small needle knife release and flexor pollicis tendon reconstruction is 15±3.8 days.All cases were followed-up and the duration is 11-20 months.No incision infection,joint stiffness,tendon adhesion,finger deformity or other complications occurred after the operation.Of them,9 fingers were excellent,5 fingers good ang 2 fingers fair by TAM score,the percentage of excellent and good was 87.5%.And 11 fingers were excellent,4 fingers good ang 1 fingers fair by Crawford score,the percentage of excellent and good was 93.8%.Conclusion:Fine clinical effect can be obtained for the repair of flexor pollicis tendon rupture caused by small knife needle associated with tenosynovitis stenosans with transferring superficial flexor tendon of the ring finger.In addition,the surgery is simple and easy to operate which is worthy of clinical application.
