平行病历在安宁疗护团队化服务中的应用

摘　　要：医务人员基于循证医学规范书写临床病历,与此对应叙事医学鼓励从医人员撰写行医时的体验,平行病历作为叙事医学重要实践载体体现了浓厚的人文关怀理念。不同专业人员共同组建安宁疗护团队为终末期患者及家属服务,除控制不适躯体症状外,还要对患者本人及家属进行心理调适、社会资源链接和灵性关怀等。撰写平行病历有助于提升安宁疗护人文关怀理念,促进团队成员协作精神并提升专业、人文素养。鼓励患者家属参与平行病历,从而让被服务对象成为叙事医学的直接参与者,有助于改善医患关系。叙事医学与循证医学应有效融合,建议在医院信息化建设中将平行病历纳入电子病历系统。Based on the standard of evidence-based medicine,medical staff write clinical medical records,which correspond to the experience of narrative medicine in encouraging medical staff to write medical practice.As an important practice carrier of narrative medicine,parallel medical records embody a strong concept of humanistic care.The hospice care team,which is jointly organized by different professionals,serves the end-stage patients and their families.In addition to controlling the uncomfortable physical symptoms,it also provides psychological adjustment,social resource link and spiritual care for the patients themselves and their families.Writing parallel medical records is helpful to improve the concept of humanistic care in hospice care,to promote team members’collaboration and professional quality.Encouraging the family members of patients to participate in the parallel medical records,so that the service objects can become the direct participants of narrative medicine,which will help to improve the doctor-patient relationship.Narrative medicine and evidence-based medicine should be effectively integrated.It is suggested that parallel medical records should be included in the electronic medical record system in the hospital information system.
