单髁置换与全膝关节置换在内侧胫股关节炎中的治疗效果及对VAS评分的影响

摘　　要：目的:单髁置换与全膝关节置换在内侧胫股关节炎中的治疗效果及对VAS评分的影响。方法:选择2017年3月-2019年5月本院收治的66例内侧胫股关节炎患者作为研究对象,以不同的治疗方式为依据将患者分为对照组(n=33)与研究组(n=33)。对照组采取全膝关节置换术,研究组应用单髁置换术。对比两组相关临床指标、VAS评分、生活质量评分以及膝关节美国特种外科医院膝关节评分(HSS评分)。结果:研究组相关临床指标均优于对照组(P<0.05)。两组治疗后VAS评分均改善,且研究组改善程度高于对照组(P<0.05);研究组治疗后各项生活质量评分均高于对照组(P<0.05);两组治疗前、治疗3个月后HSS评分比较,差异均无统计学意义(P>0.05);对照组治疗2周后HSS评分低于研究组(P<0.05)。结论:单髁置换与全膝关节置换治疗内侧胫股关节炎患者均有较为理想的临床效果,但单髁置换患者接受程度更高,VAS评分更低,降低患者在治疗过程中受到的伤害,促进术后康复与预后,可在临床推广应用。Objective:To investigate the effect of unicondylar replacement and total knee arthroplasty in medial tibiofemoral arthritis and the influence on VAS score.Method:A total of 66 patients with medial tibiofemoral arthritis admitted in our hospital from March 2017 to May 2019 were selected as the research objects,and the patients were divided into the control group(n=33)and the study group(n=33)according to the different treatment methods.The control group underwent total knee arthroplasty,and the study group underwent unicondylar arthroplasty.The related clinical indicators,VAS score,quality of life score,and knee American Hospital of Special Surgery HSS score(HSS score)were compared between two groups.Result:The relevant clinical indicators of the study group were better than those of the control group(P<0.05).The VAS scores of two groups after treatment were improved,and the improvement degree of the study group was higher than that of the control group(P<0.05).The all quality of life scores of the study group after treatment were higher than those of the control group(P<0.05).There were no significant differences in HSS scores between two groups before treatment and after 3 months of treatment(P>0.05).After 2 weeks of treatment,the HSS score of the control group was lower than that of the study group(P<0.05).Conclusion:Unicondyle replacement and total knee arthroplasty in the treatment of patients with medial tibiofemoral arthritis has an ideal clinical effect,but patients with unicondyle replacement have a higher degree of acceptance and a lower VAS score,which reduces patient’s injuries during treatment,promote the recovery and prognosis after surgery,it can be promoted and applied in clinical practice.
