胫前减张切口结合锁定加压钢板内固定术对老年胫腓骨远端骨折患者踝关节功能及并发症的影响研究

摘　　要：目的:研究老年胫腓骨远端骨折治疗中胫前减张切口结合锁定加压钢板内固定术对患者踝关节功能及并发症的影响。方法:回顾性选取2016年1月-2020年1月本院收治的老年胫腓骨远端骨折患者100例,依据手术方法分为胫前减张切口结合锁定加压钢板内固定术组(胫前减张切口组)与胫骨远端内侧小切口结合锁定加压钢板内固定术组(胫骨远端切口组),各50例。统计分析两组围术期指标、踝关节功能、临床疗效、术后并发症发生情况。结果:胫前减张切口组术后消肿治疗时间、骨折愈合时间均短于胫骨远端切口组(P<0.05)。胫前减张切口组手术后患侧踝关节疼痛、患足力线、功能评分及总分均高于胫骨远端切口组(P<0.05)。胫前减张切口组踝关节功能恢复优良率为94.00%,高于胫骨远端切口组的78.00%(P<0.05)。胫前减张切口组术后并发症发生率为18.00%,低于胫骨远端切口组的46.00%(P<0.05)。结论:老年胫腓骨远端骨折治疗中胫前减张切口结合锁定加压钢板内固定术较胫骨远端内侧小切口结合锁定加压钢板内固定术更能有效改善患者踝关节功能,减少术后并发症。Objective:To study the effect of anterior tibial decompression incision combined with locking compression plate internal fixation on ankle function and complications in elderly patients with distal tibial and fibular fracture.Method:A total of 100 cases of elderly patients with distal tibia and fibular fracture treated in our hospital from January 2016 to January 2020 were selected retrospectively.According to the operation methods,100 patients were divided into two groups:anterior tibial decompression incision combined with the locking compression plate internal fixation group(tibial decompression incision group)and small incision in the distal tibia combined with locking compression plate internal fixation group(distal tibial incision group),50 cases in each group.The perioperatic period indexes,ankle functions,clinical effects and postoperative complications of two groups were statistically analyzed.Result:The time of treatment of swelling after operation and the healing time of fracture of the anterior tibial decompression incision group were shorter than those of the distal tibial incision group(P<0.05).After operation,the scores of pain,line of foot,function scores and total score of ankle joint of the anterior tibial decompression incision group were higher than those of the distal tibial incision group(P<0.05).The excellent rate of ankle function recovery in the anterior tibial decompression incision group was 94.00%,which was higher than 78.00%in the distal tibial incision group(P<0.05).The postoperative complications rate of the anterior tibial decompression incision group was 18.00%,which was lower than 46.00%of the distal tibial incision group(P<0.05).Conclusion:In the treatment of elderly patients with distal tibial and fibular fracture,the anterior tibial decompression incision combined with locking compression plate internal fixation is more effective than small incision in the distal tibia combined with locking compression plate internal fixation in improving ankle function and reducing postoperat
