刃针松解术联合益气化瘀汤对腰椎间盘突出症患者的临床疗效观察

摘　　要：目的:探讨刃针松解术联合益气化瘀汤治疗腰椎间盘突出症(LDH)患者的效果及对血清基质金属蛋白酶-3(MMP-3)、白细胞介素-6(IL-6)水平的影响。方法:选取2019年10月-2020年6月于辽宁中医药大学附属医院收治住院的80例腰椎间盘突出症患者,根据随机数字表法将其分为中医综合治疗组和常规西医治疗组,每组40例。中医综合治疗组有2例脱落、失访,常规西医治疗组有1例脱落、失访。常规西医治疗组给予超短波治疗与腰背肌功能锻炼等常规保守治疗,中医综合治疗组则在常规治疗组基础上给予刃针松解术联合益气化瘀汤。比较两组疗效及治疗前后的VAS、ODI、JOA评分和血清MMP-3、IL-6水平。结果:中医综合治疗组总有效率为92.11%,高于常规西医治疗组的66.67%,差异有统计学意义(P<0.05)。治疗前,两组VAS、ODI、JOA评分比较,差异均无统计学意义(P>0.05);治疗后,两组VAS、ODI评分均低于治疗前,JOA评分高于治疗前,且中医综合治疗组的VAS、ODI评分均低于常规西医治疗组,JOA评分高于常规西医治疗组,差异均有统计学意义(P<0.05)。治疗前,两组MMP-3、IL-6水平比较,差异均无统计学意义(P>0.05);治疗后,两组MMP-3、IL-6水平均低于治疗前,且中医综合治疗组均低于常规西医治疗组,差异均有统计学意义(P<0.05)。结论:刃针松解术联合益气化瘀汤治疗腰椎间盘突出症患者临床效果较好,可有效缓解患者疼痛症状,提高腰椎活动功能,降低血清MMP-3、IL-6水平,值得临床进一步推广应用。Objective:To discuss clinical efficacy of release with small needle knife combined with Yiqi Huayu Decoction on lumbar disc herniation(LDH)and influence on serum matrix metalloproteinase 3(MMP-3)and interleukin-6(IL-6).Method:A total of 80 patients with lumbar disc herniation admitted to Affiliated Hospital of Liaoning University of traditional Chinese Medicine from October 2019 to June 2020 were selected,and they were randomly divided into TCM comprehensive treatment group and conventional western medicine treatment group according to the random number table method,40 patients in each group.There were two cases of abscission and loss of follow-up in the TCM comprehensive treatment group and 1 case of conventional western medicine treatment group.The conventional western medicine treatment group was given ultrashort wave therapy and lumbar and back muscle function exercise and other conventional conservative treatment,the TCM comprehensive treatment group was given release with small needle knife combined with Yiqi Huayu Decoction on the basis of conventional treatment group.The efficacy was compared between two groups,VAS,ODI,JOA scores,serum MMP-3 and IL-6 levels before and after treatment were compared between two groups.Result:The total effective rate of TCM comprehensive treatment group was 92.11%,higher than 66.67% of conventional western medicine treatment group,the difference was statistically significant(P<0.05).Before treatment,there were no significant differences in VAS,ODI and JOA scores between two groups(P>0.05);after treatment,VAS and ODI scores of both groups were lower than those before treatment,and JOA scores were higher than those before treatment,VAS and ODI scores of TCM comprehensive treatment group were lower than those of conventional western medicine treatment group,and JOA score was higher than that of conventional western medicine treatment group,the differences were statistically significant(P<0.05).Before treatment,there were no significant differences in the levels of MMP-3 and IL-
