摘　　要：目的探讨血清鸢尾素(irisin)水平对多囊卵巢综合征(PCOS)患者的诊断价值及与PCOS发生的相关性。方法选取2018年1月至2019年1月淮安市妇幼保健院收治的80例PCOS患者纳入PCOS组,另选取80例进行体检的健康女性纳入对照组。应用受试者操作特征(ROC)曲线分析血清irisin水平在PCOS患者诊断中的价值。采用Pearson相关分析及多元逐步回归分析血清irisin与基线资料的相关性。采用Logistic回归分析血清irisin与PCOS发生的相关性。结果PCOS组irisin水平高于对照组,差异具有统计学意义(P<0.05)。ROC曲线结果显示,血清irisin诊断PCOS患者的最佳截断值为2.45ng/L。PCOS患者irisin水平与睾酮(T)、稳态模型胰岛素抵抗指数(HOMA-IR)呈正相关(P<0.05)。多元逐步回归分析显示,卵泡刺激素(FSH)、黄体生成素(LH)、T、平均卵巢体积(MOV)是影响irisin水平的重要因素(P<0.05)。二分类Logistic回归分析结果显示,LH升高、雌二醇(E2)降低、T升高、irisin升高是影响PCOS发生的危险因素。结论irisin可作为PCOS患者血清学诊断标志物,且高水平irisin与PCOS发生密切相关。Objective To investigate the diagnostic value of serum irisin level in patients with polycystic o-vary syndrome(PCOS)and its correlation with the occurrence of PCOS.Methods 80 cases of PCOS patients admitted to Huai'an Maternal and Child Health Hospital from January 2018 to January 2019 were included in the P-COS group,and another 80 cases of healthy women undergoing physical examination were included in the control group.Receiver operating characteristic(ROC)curve was used to analyze the diagnostic value of serum irisin level in patients with PCOS.Pearson correlation analysis and multiple stepwise regression were used to analyze the correlation between serum irisin and baseline data.Logistic regression was used to analyze the correlation between serum irisin and PCOS.Results The level of irisin in PCOS group was higher than that in control group(P<0.05).ROC curve results showed that the optimal cut-off value of serum irisin in the diagnosis of PCOS patients was 2.45ng/L.Irisin level in PCOS patients was positively correlated with testosterone(T)and homeostasis model insulin resistance index(HOMA-IR)(P<0.05).Multiple stepwise regression analysis showed that follicle stimulating hormone(FSH),luteinizing hormone(LH),T and mean ovarian volume(MOV)were important factors affecting the level of irisin(P<0.05).The results of binary Logistic regression analysis showed that the increase of LH,the decrease of estradiol(E2),the increase of T and the increase of irisin were the risk factors affecting the occurrence of PCOS.Conclusions Serum irisin can be used as a serological marker in patients with diagnosed PCOS,and the high level of irisin is closely related to the occurrence of PCOS.
