摘　　要：目的采用清浊汤治疗湿热瘀阻型慢性前列腺炎合并弱精症,观察其临床疗效,探讨其可能机制。方法选取2017年1月至2019年10月沭阳县中医院诊治的66例湿热瘀阻型慢性前列腺炎合并弱精症患者作为研究对象。随机分治疗组和对照组,各33例。对照组给予口服泽桂癃爽胶囊治疗,治疗组给予中药清浊汤煎服。比较两组治疗前后症状改变及美国国立卫生研究院慢性前列腺炎症状指数(NIH-CP-SI)改善情况。比较治疗前后前列腺液常规、精液常规及精子DNA碎片指数(DFI)的变化。结果治疗后,治疗组NIH-CPSI评分低于对照组,差异具有统计学意义(P<0.05);治疗组总有效率高于对照组,差异具有统计学意义(P<0.05);治疗组精液质量指标优于对照组,差异具有统计学意义(P<0.05)。结论清浊汤治疗湿热瘀阻型慢性前列腺炎合并弱精症疗效显著,其机理可能与前列腺液中白细胞数明显减少,炎症刺激减轻,精液液化时间缩短,患者情志及生活质量改善等相关。Objective To observe the effects of Qingzhuo Decoction on sperm quality in patients with chronic prostatitis combined with asthenospermia and its mechanism.Methods 66 patients with dampness-heat stasis chronic prostatitis complicated with aspermia diagnosed and treated in Shuyang County Traditional Chinese Medicine Hospital from January 2017 to October 2019 were selected and randomly divided into treatment group and control group,with 33 cases in each group.The control group was given oral treatment with Zeguilongshuang capsule,and the treatment group was given Qingzhuo Decoction.Symptomatic changes and improvements in NIH-CPSI before and after treatment were compared between the two groups.The changes of prostatic fluid routine,semen routine and sperm DNA fragment index(DFI)were compared before and after treatment.Results After treatment,the NIH-CPSI score of the treatment group was lower than that of the control group(P<0.05).The total effective rate in treatment group was higher than that in control group(P<0.05).Semen quality in the treatment group was better than that in the control group(P<0.05).Conclusions Qingzhuo Decoction has a significant effect on dampness and heat stasis type chronic prostatitis complicated with aspermia,and its mechanism may be related to the significant reduction of white blood cell count in prostatic fluid,the alleviation of inflammatory stimulation,the shortening of semen liquefaction time,and the improvement of patients'mood and quality of life.
