摘 要:目的:研究双源CT(dual source computed tomography,DSCT)后处理技术在孤立性肺结节(solitary pulmonary nodule,SPN)良恶性诊断价值。方法:回顾性收集2018年7月-2020年8月在本院进行治疗的96例SPN患者的病例资料。所有患者手术前均进行DSCT平扫及增强扫描,将原始图像传输到工作站进行后处理操作,比较CT扫描原始图像和后处理图像的诊断结果,并分析与金标准的一致性。结果:96例SPN患者中,良性结节39例,恶性结节57例,CT扫描原始图像正确诊断恶性结节患者33例,后处理图像正确诊断恶性结节患者54例,后处理图像的分叶征、毛刺征、血管集束征、胸膜凹陷征、支气管截断征、空泡征显示率均高于CT扫描原始图像(P<0.05),CT扫描原始图像与金标准一致性较差(Kappa=0.374),后处理图像与金标准具有较好的一致性(Kappa=0.848),后处理图像诊断SPN良恶性的灵敏度、准确率、阴性预测值均高于CT扫描原始图像(P<0.05)。结论:DSCT后处理技术在SPN良恶性诊断中具有较高的价值。Objective:To study the value of dual source computed tomography(DSCT)post-processing technology in the diagnosis of benign and malignant solitary pulmonary nodule(SPN).Method:The clinical data of 96 patients with SPN treated in our hospital from July 2018 to August 2020 were collected retrospectively.All patients underwent DSCT plain scan and enhanced scan before operation.The original image was transmitted to the workstation for post-processing.The diagnostic results of the original image and the post-processing image of CT scan were compared and the consistency was compared with the gold standard.Result:Among the 96 SPN patients,39 were benign nodules,57 were malignant nodules,33 patients were correctly diagnosed with malignant nodules on CT scan original images,and 54 patients were correctly diagnosed with malignant nodules by post-processed images.The display rates of lobular sign of post-processed images such as burr sign,vascular cluster sign,pleural depression sign,bronchus truncation sign and vacuole sign were all higher than those of the original CT scan image(P<0.05).The original CT scan image has poor consistency with the gold standard(Kappa=0.374),and the post-processed image has good consistency(Kappa=0.848).The sensitivity,accuracy,and negative predictive value of post-processing images in the diagnosis of SPN benign and malignant were higher than those of the original images of CT scans(P<0.05).Conclusion:SCT post-processing technology has high value in the diagnosis of SPN benign and malignant.
