人工流产术后不同时机放置左炔诺孕酮宫内节育系统对再次人工流产率及生殖道感染的影响

摘　　要：目的:探讨人工流产术后不同时机放置左炔诺孕酮宫内节育系统(LNG-IUS)对再次人工流产率及生殖道感染的影响。方法:选取2018年1月-2019年12月本院人工流产的患者198例,随机分为对照组和研究组,各99例。对照组在月经后4~7 d月经量少许的时候放置LNG-IUS,研究组术后即刻放置LNG-IUS,观察两组不良反应和再次人工流产率、生殖道感染情况及放置半年、1年后续用率和阴道点滴出血的情况。结果:研究组下腹痛、面部痤疮、乳胀、闭经等不良反应发生率低于对照组(P<0.05),研究组再次人工流产率低于对照组(P<0.05), 研究组生殖道感染发生率低于对照组(P<0.05), 两组放置半年、1年的续用率对比差异无统计学意义(P>0.05), 研究组放置半年、1年的阴道点滴出血率均低于对照组(P<0.05)。结论:人工流产术后,相较于月经后放置LNG-IUS,即时放置效果更佳,可以有效降低生殖道的感染情况,降低再次人工流产率,且即时放置后阴道点滴出血率较低,有利于患者的康复。Objective:To explore the influences of levonorgestrel intrauterine contraceptive system(LNG-IUS)placed at different times after artificial abortion on the rate of re-induced abortion and reproductive tract infection.Method:A total of 198 patients with artificial abortion in our hospital from January 2018 to December 2019 were selected and randomly divided into the control group and the study group.The control group was placed with LNG-IUS 4-7 d after menstruation when the menstrual volume was a little bit,and the study group was placed with LNG-IUS immediately after the operation.The adverse reactions and re-induced abortion rate,the reproductive tract infection and continuation and vaginal spotting rates for 6 months and 1 year of the two groups were observed.Result:The incidence of adverse reactions such as lower abdominal pain,facial acne,breast swelling and amenorrhea in the study group was lower than that of the control group(P<0.05);the rate of re-induced abortion in the study group was lower than that in the control group(P<0.05);the incidence of reproductive tract infection in the study group was lower than that in the control group(P<0.05);there were no statistically significant differences in the continuation rate between the two groups after half a year and 1 year(P>0.05);the rates of vaginal spotting after half a year and 1 year in the study group were lower than those of the control group(P<0.05).Conclusion:After artificial abortion,compared with placing LNG-IUS after menstruation,the effect of immediate placement is better,which can effectively reduce the infection of the reproductive tract and reduce the rate of artificial re-abortion,and the rate of vaginal spotting after immediate placement is lower,which is beneficial to patients’recovery.
