摘　　要：延续性护理是指患者自医院回到家庭或社会后的延伸护理行为。对于回到养老机构的患者,延续性护理主要内容为多学科团队向患者及护理员提供专业的指导和支持,使患者在养老机构中得到个性化的护理。本文旨在分析概括国内外对延续性护理概念的定义,对国内外养老机构中的延续性护理模式及研究进展进行综述,为我国延续性护理在养老机构中的发展提供参考建议。Continuous nursing refers to the extended nursing behavior of patients after they return to family or society from the hospital.For the patients who return to the nursing home,the main content of continuity nursing is to provide professional guidance and support to patients and nurses by multidisciplinary team,so that patients can get personalized nursing in the nursing home.The purpose of this paper is to analyze and summarize the definition of continuity nursing at home and abroad,summarize the continuity nursing mode and research progress in pension institutions at home and abroad,and provide reference and suggestions for the development of continuity nursing in pension institutions in China.
