摘 要:目的:探讨吸入式七氟醚全麻对体外循环下心脏瓣膜置换术患者心肌肌钙蛋白I(cTn-I)水平的影响。方法:选取笔者所在医院2018年1月-2021年1月收治的82例应用心脏瓣膜置换术的患者作为研究对象,采用随机数字表法将其分为两组,每组41例。对照组应用静脉式丙泊酚全麻,研究组应用吸入式七氟醚全麻。比较两组不同时间cTn-I水平、肌酸激酶同工酶(CK-MB)水平及炎性水平。结果:研究组T1、T2、T3时cTn-I水平均低于对照组,差异有统计学意义(P<0.05);研究组T1、T2、T3时CK-MB水平均低于对照组,差异有统计学意义(P<0.05);研究组T1、T2、T3时IL-6、IL-8水平均低于对照组,差异有统计学意义(P<0.05)。结论:体外循环下心脏瓣膜置换术患者应用吸入式七氟醚全麻的麻醉方式,可抑制心肌酶的释放,降低炎性水平,以达到保护心肌的目的,具有极大的应用价值。Objective:To investigate the effect of inhaled Sevoflurane general anesthesia on cardiac troponin I(cTn-I)level in patients undergoing cardiac valve replacement under cardiopulmonary bypass.Method:A total of 82 patients received cardiac valve replacement in our hospital from January 2018 to January 2021 were selected as the research objects,and they were divided into two groups by random number table method,with 41 patients in each group.The control group received intravenous Propofol anesthesia,and the study group received inhaled Sevoflurane general anesthesia.The levels of cTn-I,creatine kinase isoenzyme(CK-MB)and inflammation at different time were compared between the two groups.Result:The levels of cTn-I at T1,T2 and T3 in the study group were lower than those in the control group,the differences were statistically significant(P<0.05).The level of CK-MB at T1,T2 and T3 in the study group were lower than those in the control group,the differences were statistically significant(P<0.05).The levels of IL-6 and IL-8 in the study group were lower than those in the control group at T1,T2 and T3,and the differences were statistically significant(P<0.05).Conclusion:Inhaled Sevoflurane general anesthesia for patients undergoing cardiac valve replacement under cardiopulmonary bypass can inhibit the release of myocardial enzymes and reduce the inflammatory level,so as to achieve the purpose of myocardial protection,which has great application value.
