摘　　要：目的:探讨输尿管镜钬激光碎石术(URSL)治疗不同位置输尿管结石的临床效果与安全性。方法:选择2016年6月-2020年6月本院接受URSL手术治疗的输尿管结石患者83例进行观察分析,根据患者结石位置差异分为输尿管上段结石组(23例)、输尿管中段结石组(30例)、输尿管下段结石组(30例)。对比三组临床治疗效果,记录各组患者手术相关指标,统计患者术后出现并发症的概率。结果:输尿管上段结石组治疗总有效率为95.65%,输尿管中段、下段结石组治疗有效率均为96.67%,输尿管中、下段结石组手术治疗效果优于输尿管上段结石组,三组治疗总有效率对比差异无统计学意义(P>0.05),三组相关手术指标、术后并发症发生率差异无统计学意义(P>0.05)。结论:输尿管结石行URSL手术治疗具有显著效果,安全性佳,尤其针对中下段输尿管结石患者,可用作首选治疗方案。Objective:To investigate the clinical effect and safety of ureteroscopic holmium laser lithotripsy(URSL)in the treatment of ureteral calculi at different positions.Method:Eighty-three patients with ureteral calculi who received URSL surgery in our hospital from June 2016 to June 2020 were selected for observation and analysis,and were divided into the upper ureteral calculi group(23 cases),the middle ureteral calculi group(30 cases),the lower ureteral calculi group(30 cases)according to the difference in the location of calculi.The clinical treatment effect of patients in the three groups was compared,relevant surgical indicators of patients in each group were recorded,and the incidence of postoperative complications was counted.Result:The total effective rate of the upper ureteral calculi group was 95.65%,and the effective rates of the middle and lower ureteral calculi groups were 96.67%,the surgical treatment effect of the middle and lower ureteral calculi groups was better than that of the upper ureteral calculi group,there was no significant difference in the total effective rate among the three groups(P>0.05),and there was no statistically significant difference in the relevant surgical indicators and the incidence of postoperative complications among the three groups(P>0.05).Conclusion:URSL surgery for ureteral calculi has significant effect and good safety.Especially for patients with middle and lower ureteral calculi, URSL can be used as the first choice of treatment.
