临床药师参与用药咨询服务的实践与分析

摘　　要：目的分析用药咨询服务的现状,为促进用药咨询和药学服务发展提供参考。方法回顾性分析福建省泉州市第一医院临床药师参与用药咨询服务的实践情况,分析咨询问题数量、咨询者年龄、咨询内容及咨询药物类别。结果2016-2019年接待患者咨询5776例次,其中药学专业问题1379例次占比23.87%。临床药师参与用药咨询服务后,咨询问题数量从2016年898例次提高至2019年2203例次(x^(2)=297.771,P=0.000),药学专业问题咨询率从11.02%提高至38.86%(x^(2)=321.865,P=0.000);咨询者5776例次中,36~45岁占比最高为32.79%,而>65岁仅占12.83%;药学专业问题以用法用量、药效或药理作用及特殊剂型用药指导为主,分别为375例次(27.19%)、292例次(21.17%)和258例次(18.71%)。患者咨询最多的前3类药物分别为消化系统药物、心血管系统药物、抗菌药物。结论临床药师参与用药咨询可有效解决患者用药疑问,有助于改善用药咨询服务质量,保障患者用药安全,促进合理用药。Objective To analyze the current situation of pharmaceutical consultation service and provide reference for promoting the development of pharmaceutical consultation and pharmaceutical service.Methods The practice of clinical pharmacists participating in pharmaceutical consultation service in Quanzhou First Hospital of Fujian Province was analyzed retrospectively,and the number of consultation questions,the patientsfage,the consulting contents and the types of drug were statistically analyzed.Results 5776 patients consultations from 2016 to 2019 were received,among them,1379 cases of pharmacy professional questions accounted for 23.87%.After clinical pharmacists participated in medication consultation services,the number of consultation questions increased from 898 cases in 2016 to 2203 cases in 2019(x^(2)=297.771,P=0.000),and the consultation rate of pharmacy professional problems increased from 11.02%to 38.86%(x^(2)=321.865,P=0.000).Among the 5776 interviewees,the 36〜45 years old accounted for the highest proportion of 32.79%,while the>65 years old accounted for only 12.83%.The pharmacy professional issues were mainly based on usage and dosage,pharmacological effects or pharmacological effects and special dosage forms of medication guidance,which were 375 cases(27.19%),292 cases(21.11%)and 258 cases(18.71%).The top 3 drugs that patients consulted most were digestive system drugs,cardiovascular system drugs and antibacterial drugs.Conclusion The participation of clinical pharmacists in pharmaceutical consultation can solve the questions of patients,medication,improve the quality of pharmaceutical consultation service,ensure the safety in drug use and promote rational drug use.
