摘　　要：目的探讨具有乳头样核特征的非浸润性甲状腺滤泡性肿瘤(NIFTPs)的超声及病理特征,并与甲状腺腺瘤(TAs)鉴别诊断。方法回顾性对比分析经手术病理证实的34例NIFTPs(9例行FNA)、47例TAs(7例行FNA)的超声表现,并观察两者的FNA结果。结果两类结节在内部回声及血流分布情况等方面差异有统计学意义(P<0.05),NIFTPs较TAs更易表现为等回声、仅结节内部存在血流。9例行FNA的NIFTPs患者中,7例诊断错误,其中5例诊断为结节性甲状腺肿可能,2例诊断为甲状腺乳头状癌可能。7例行FNA的TAs患者中,1例诊断错误,误诊为结节性甲状腺肿可能。结论 NIFTPs的超声表现存在一定特征,倾向于诊断为良性结节,目前FNA难对其进行精确诊断,其定性诊断依靠手术病理。Objective To investigate the ultrasonographic and pathological features of noninvasive follicular thyroid neoplasm with papillary-like nuclear features(NIFTPs),and to explore the diagnostic value in differentiating with thyroid adenomas(TAs).Methods Retrospective comparative analysis was performed on the ultrasonographic features of 34 NIFTPs(9 NIFTPs underwent FNA) and 47 TAs(7 TAs underwent FNA),whose diagnosis were confirmed via surgical pathology from in the First Affiliated Hospital of Nanchang University, and observe the FNA results of all. Results There were significant differences in the internal echo and blood flow distribution between the two types of nodules(all P<0.05). NIFTPs was more likely to be isoechoic and only blood flow existed in the nodules than TAs. Of the 9 patients with NIFTPs who underwent FNA, 7 were misdiagnosed, 5 of them were diagnosed as nodular goiter, 2 of them were diagnosed as papillary thyroid carcinoma.Of the 7 patients with TAs who underwent FNA, 1 was misdiagnosed as nodular goiter. Conclusions The ultrasound findings of NIFTPs have some characteristics and tend to be diagnosed as benign nodules.At present, FNA is difficult to make accurate diagnosis of niftps. The qualitative diagnosis of NIFTPs depends on surgical pathology.
