摘  要:目的探讨剪切波黏弹性技术在鉴别激光消融治疗兔肝VX2肿瘤后癌残留与炎症反应的价值。方法制作兔肝VX2肿瘤部分消融模型,在消融治疗后3、7、14 d对肿块区进行超声检查,比较残癌组织与消融后炎症反应带的剪切波黏弹性值差异。结果 15只VX2兔肝肿瘤成功建立部分消融模型。消融术后第7天残癌剪切波弹性值及频散值达到最高,分别为(26.06±13.14)kPa及(17.73±4.64)(m/s)/kHz(P<0.05)。残癌在各时间段剪切波弹性值均高于炎症反应带(P<0.05);而炎症反应带的剪切波频散值在消融术后第7天及第14天高于残癌(P<0.05)。结论超声剪切波黏弹性技术无创、灵敏、可重复检查,对消融术后残癌及炎症的鉴别有一定应用价值。Objective To explore the value of shear wave elastography(SWE) combined with shear wave dispersion(SWD) in differentiating the residual cancer from inflammatory response tissue after laser ablation of hepatocellular carcinoma in VX2 rabbits. Methods Ultrasonic examinations were performed at 3, 7, and 14 days after partial ablation of hepatocellular carcinoma in VX2 rabbits. The difference in SWE and SWD values between the residual tumor and the inflammatory response tissue were compared. Results Partial ablation models of hepatocellular carcinoma were established in 15 VX2 rabbits. The peak values of SWE and SWD of residual cancer on the 7 th day after ablation were(26.06±13.14)kPa and(17.73±4.64)(m/s)/kHz respectively(P<0.05). Compared with inflammatory response tissue, residual cancer had the higher value of SWE at 3, 7, and 14 days after partial ablation(P<0.05);while the value of SWD in inflammatory response tissue was higher than that of residual cancer at 7 and 14 days after ablation(P<0.05). Conclusions As a non-invasive, sensitive and repeatable technique, the combined measurement of SWE and SWD has a certain clinical significance in differentiation of residual cancer and inflammation response tissue after ablation.
