摘 要:目的探讨超声引导下射频消融治疗局限性结核病变的安全性及有效性。方法回顾分析16例行超声引导下射频消融治疗的局限性结核病变患者。统计消融术中及术后的并发症,分析射频消融治疗局限性结核病变的安全性。术后1、3、6、9、12、24个月,患者行影像学检查,分析其有效性。结果术中及术后所有患者均未出现周围器官损伤、针道种植及结核播散等严重并发症。术后1个月,16例患者的消融灶(23个)体积较术前不同程度缩小,术后24个月消融灶缩小率与术后3、6、9个月比较,差异均具有统计学意义(P<0.05);与术后12个月比较,差异无统计学意义(P>0.05)。随访至今,所有患者均未发现活动性进展或复发。结论超声引导下射频消融治疗局限性结核病变微创、安全、有效,值得临床推广应用。Objective To investigate the safety and efficacy of ultrasound-guided percutaneous radiofrequency ablation for localized tuberculosis. Methods A retrospective analysis was made of 16 patients with localized tuberculosis undergoing ultrasound-guided radiofrequency ablation in the Department of Ultrasound of the First Affiliated Hospital of Zhengzhou University.The complications during and after ablation were counted to analyze the safety of radiofrequency ablation in the treatment of localized tuberculosis.At 1, 3, 6, 9, 12, and 24 months after ablation, patients underwent enhanced MRI and contrast-enhanced ultrasound to analyze its effectiveness. Results No serious complications such as peripheral organ injury, needle canal implantation and tuberculosis spread occurred in all patients during and after operation.At the first month after surgery, the volume of ablation lesions in the 16 patients(23) was reduced to different degrees compared with that before surgery, and the reduction rate of ablation lesions at 24 months after surgery was statistically significant compared with that at 3, 6 and 9 months after surgery(P<0.05). Compared with 12 months after surgery, the difference was not statistically significant(P> 0.05). Conclusions Ultrasound-guided radiofrequency ablation for localized tuberculosis is minimally invasive, safe and effective, and worthy of clinical application.
