血清尿酸/肌酐比值与早期糖尿病肾病的相关性探讨

摘　　要：目的探讨在2型糖尿病(T2DM)患者中探讨血清尿酸/肌酐(UA/Cr)与早期糖尿病肾病(DN)的关系。方法选取单纯T2DM组(n=160)和早期DN组(n=140),另选取健康体检者150例为对照组,对所有入选者进行尿白蛋白/肌酐比值(ACR)、血糖、血脂、肾功能等测定,计算UA/Cr比值。结果早期DN组UA/Cr显著高于单纯T2DM组和对照组(P<0.05);Pearson相关分析提示,ACR与UA/Cr呈正相关(r=0.240,P<0.001);Logistic回归分析提示UA/Cr升高是早期DN发生的独立危险因素(P=0.006);ROC曲线分析提示UA/Cr对早期DN的发生有较好的预测价值,曲线下面积为0.787(95%CI:0.719~0.855,P<0.001),优于血清UA对早期DN的预测价值。结论UA/Cr是早期DN发生的独立危险因素,可作为预测早期DN发生的一种临床指标。Objective To investigate the correlation between serum uric acid/creatinine(UA/Cr)and early diabetic nephropathy(DN)in patients with type 2 diabetes mellitus(T2DM).Methods T2DM patients without DN(n=160)and patients with early DN(n=140)were enrolled in our study.In addition,150 healthy subjects were selected as the control group.Urinary albumin/creatinine ratio(ACR),blood glucose,lipid profile and kidney function were measured for all the selected subjects,and the UA/Cr ratio was calculated.Results UA/Cr in the early DN group was significantly higher than that in T2DM group and control group(P<0.05).Pearson correlation analysis suggested that ACR was positively correlated with UA/Cr(r=0.240,P<0.001).Logistic regression analysis showed that UA/Cr elevation was an independent risk factor for the occurrence of early DN(P=0.006).ROC curve analysis suggested that UA/Cr had a predictive value for the occurrence of early DN,with the area under the curve was 0.787(95%CI:0.719-0.855,P<0.001),which was larger than the predictive value of serum UA.Conclusion UA/Cr is an independent risk factor for early DN and may be used as a useful predictor for early DN.
