摘 要:目的分析移居高原低氧环境下男性医务人员在不同时期的性激素水平、精液质量和性功能改变。方法选取2017年8月至2018年5月湖南各省直及市县单位的30例进行援藏(海拔3 500m)且入藏前精液质量、性激素水平、性功能勃起都正常的男性医务人员(试验组)作为研究对象。另选取2017年8月至2018年5月湖南中医药大学附属宁乡医院的30例正常上班的男性医务人员(平原)作为对照组。研究对象在参与调查前的生殖系统各项指标均正常,分析试验组在3个月、6个月、9个月及返回平原地区后的性激素水平、精液质量和性功能改变情况。结果与对照组比较,试验组进入高原低氧环境后性激素、精液质量和性功能评分均下降,差异具有统计学意义(P<0.05);随着进入高原低氧环境时间的增加,试验组血清T、E2水平、精子密度、精子畸形率显著升高,精子活动率和性功能评分降低,差异具有统计学意义(P<0.05)。试验组返回平原地区后,随着时间的增加,性激素、精液质量和性功能评分均有所恢复,差异具有统计学意义(P<0.05);在返回平原地区3个月时,试验组血清E2恢复至正常水平;在返回平原地区6个月时,试验组精子密度、精子活动率及性功能评分已恢复至正常水平;在返回平原地区9个月时,试验组血清T和精子畸形率恢复至正常水平。结论相对短期入藏(如3~9个月),高原低氧环境会对男性性激素水平、精液质量和性功能有不良影响,并且这些不良影响是可逆的。Objective To analyze the changes in sex hormone level, semen quality and sexual function of male medical staff at different periods in the hypoxic environment of the migration plateau. Methods 30 male medical staff(experimental group) who had aided Tibetan(altitude 3,500 m) from August 2017 to May 2018 and had normal semen quality, sex hormone level and erectile function before Tibetan aid in Hunan provincial and municipal units were selected. 30 male medical staff(Pingyuan) who had been working in Ningxiang Hospital Affiliated to Hunan University of Traditional Chinese Medicine from August 2017 to May 2018 were selected as the control group. All the indicators of the reproductive system of the subjects were normal before the survey. The changes of sex hormone levels, semen quality and sexual function in the experimental group were analyzed at 3, 6, 9 months and after returning to the plain area. Results Compared with the control group, the sex hormone, semen quality and sexual function scores in the experimental group decreased after entering the plateau hypoxia environment,(P<0.05). With the increase of time in the plateau hypoxia environment, serum T, E2 levels, sperm density and sperm malformation rate in experimental groups were significantly increased, while sperm motility rate and sexual function score were decreased(P<0.05). After the experimental group returned to the plain area, the sex hormone, semen quality and sexual function score were recovered with the increase of time(P<0.05). The serum E2 of the experimental group returned to normal level after 3 months of return to the plain area. Six months after returning to the plain area, the sperm density, sperm motility rate and sexual function score of the test group had returned to normal level. At 9 months after returning to the plain area, serum T and sperm malformation rate of the experimental group returned to normal level.
