摘 要:目的探讨小剂量低分子肝素对妊娠早期子宫动脉血流频谱异常的孕妇发生子痫前期(PE)的影响。方法选取2016年1月至2019年11月安徽省宣城市中心医院及安徽省芜湖市戈矶山医院收治的82例于妊娠11~14周子宫动脉血流频谱异常的孕妇作为研究对象。根据孕妇是否于妊娠13周至预产期前1周应用小剂量肝素分为研究组(n=42)和对照组(n=40)。比较两组孕期舒张压、收缩压、24h尿蛋白定量、PE发生率及子宫动脉血流阻力指数(RI)变化。结果研究组孕中期、孕晚期舒张压、收缩压及24h尿蛋白定量值均低于对照组,差异具有统计学意义(P<0.01)。研究组PE发生率为7.14%,低于对照组的35.00%,差异具有统计学意义(P<0.05)。两组治疗后RI值均下降,且研究组RI值下降幅度优于对照组,差异具有统计学意义(P<0.05)。结论子宫动脉血流频谱异常孕妇给予小剂量低分子肝素,可较好地预防PE发生。Objective To investigate the effect of low-dose low-molecular-weight heparin on the occurrence of pre-eclampsia(PE) in pregnant women with abnormal uterine artery blood flow spectrum in early pregnancy. Methods 82 pregnant women with abnormal uterine artery blood flow spectrum who were admitted to Xuancheng City Central Hospital and Wuhu City Gejishan Hospital from January 2016 to November 2019 were selected as the research subjects. Pregnant women were divided into study group(n=42) and control group(n=40) according to whether they had received low-dose heparin from 13 weeks of gestation to 1 week before their expected date of birth. The changes of diastolic blood pressure, systolic blood pressure, 24 h urinary protein, PE incidence and uterine arterial blood flow resistance index(RI) were compared between the two groups during pregnancy. Results The diastolic pressure, systolic pressure and 24-hour urine protein quantity of the study group were lower than those of the control group(P<0.01). The incidence of PE was 7.14% in the study group and 35.00% in the control group(P<0.05). The RI value of both groups decreased after treatment, and the RI value of uterine artery blood flow in the study group was lower than that in the control group(P<0.05). Conclusions Low-dose low-molecular-weight heparin can prevent PE in pregnant women with abnormal uterine artery blood flow spectrum.
