摘  要:“土壅木郁”是脾病及肝的一种病理过程,在慢性乙型肝炎的发病机制中起到重要作用。近年研究发现,肠道菌群结构的改变及由此引起的免疫反应与慢性乙型肝炎的发生发展相关。文章通过探讨肠道菌群失调与病理状态“土壅”的关系,从现代医学的角度,对中医学“土壅木郁”理论在慢性乙型肝炎中发病机制中的作用进行了新的解读,并对该理论的生物学基础与科学内涵进行了补充,提示调整肠道菌群结构在治疗慢性乙型肝炎中的意义,以期为临床治疗慢性乙型肝炎提供新思路。‘Earth stagnation,wood depression’,a pathological process of spleen affecting liver,plays an important role in the pathogenesis of chronic hepatitis B.Recent studies have found that the structural changes in the gut microbiota and the immune response caused by these changes are related to occurrence and development of chronic hepatitis B.Through discussing the relationship between gut microbial dysbiosis and the pathological state of‘earth stagnation’,this article put a new interpretation on the relationship between the‘earth stagnation,wood depression’theory and the pathogenesis of chronic hepatitis B from the perspective of modern medicine,complement the theory’s biological basis and scientific connotation,and suggest the significance of adjusting the gut microbiota structure in the treatment of chronic hepatitis B,in order to provide new ideas for clinical treatment of chronic hepatitis B.
