摘 要:目的:研究蜂针对胶原诱导性关节炎(CIA)大鼠Th17/Treg细胞平衡的影响,探讨蜂针治疗类风湿关节炎的作用机制。方法:50只SD大鼠随机分为:正常组、模型组、蜂针组、甲氨蝶呤组和蜂针+甲氨蝶呤组,每组10只。正常组不作处理;其余各组大鼠建立CIA模型后,分别予0.9%氯化钠溶液、活蜂、甲氨蝶呤及蜂针联合甲氨蝶呤干预,共治疗4周。每周对各组大鼠关节炎指数(AI)进行评分,治疗结束后HE染色观察踝、膝关节病理表现并进行病理损伤半定量分析;流式细胞术分析外周血Th17、Treg细胞比例;实时定量荧光PCR测定脾脏特异性转录因子RORγt、Foxp3 mRNA表达;ELISA法检测血清细胞因子IL-17、TGF-β浓度。结果:与模型组比较,各治疗组大鼠关节炎指数降低,踝、膝关节滑膜及软骨病理损伤缓解,蜂针组大鼠病理积分显著下降(P<0.05);各治疗组大鼠Th17细胞数量下降(P<0.05),蜂针组Treg细胞数量上升(P<0.05);各治疗组大鼠RORγt mRNA表达下降(P<0.05),Foxp3 mRNA表达上升(P<0.05);蜂针+甲氨蝶呤组大鼠IL-17浓度下降(P<0.05),各治疗组TGF-β浓度上升(P<0.05)。结论:蜂针能够改善CIA,其作用机制可能与调节Th17/Treg细胞的数量,影响RORγt、Foxp3 mRNA表达和IL-17、TGF-β浓度有关。Objective:To study the effects of bee venom acupuncture(BVA)on Th17/Treg balance in rats with collageninduced arthritis(CIA),and explore the mechanism of BVA in treating rheumatoid arthritis.Methods:A total of 50 SD rats were randomly divided into five groups:normal group,model group,BVA group,methotrexate group and BVA+methotrexate group,10 in each group.Rats in normal group received no treatment,while the other four groups received saline,bee venom,methotrexate and bee venom plus methotrexate treatment respectively for 4 weeks after established CIA models.The severity of CIA was evaluated by arthritis index every week.When treatment finished,ankle and knee joints histopathological changes were analyzed and scored by staining with hematoxylin-eosin.The percentages of Th17 and Treg cells in peripheral blood were analyzed by flow cytometry;the mRNA expression of RORγt and Foxp3 in spleen were analyzed by Real-time PCR method;the concentrations of serum cytokines IL-17 and TGF-βwere determined by ELISA.Results:Compared with the model group,the treatment groups reduced the arthritis index and ameliorated the damage of synovial membrane and cartilage of ankle and knee joints;pathological score of BVA group was decreased(P<0.05).The percentage of Th17 cells in treatment groups were reduced,the percentage of Treg cells in BVA group was increased(P<0.05).In treatment groups,the expressions of RORγt mRNA were reduced and Foxp3 mRNA were increased(P<0.05).BVA+methotrexate group reduced the concentration of IL-17(P<0.05),while the treatment groups increased the concentration of TGF-β(P<0.05).Conclusion:BVA ameliorates collagen-induced arthritis in rats via restoration of the balance of Th17/Treg by regulating the number of Th17,Treg cells,the expression of RORγt,Foxp3 mRNA and the concentration of IL-17 and TGF-β.
