拈痛分子中药霜剂治疗膝关节骨性关节炎的临床疗效研究

摘　　要：目的:探讨拈痛分子中药霜剂治疗膝关节骨性关节炎(KOA)的临床疗效。方法:纳入Ke l l g r e n-LawrenceⅠ~Ⅱ级KOA患者随机分为两组,治疗组85例(98膝)采用拈痛分子中药霜剂外用联合口服盐酸氨基葡萄糖胶囊治疗,对照组83例(96膝)采用双氯芬酸二乙胺乳胶剂外用联合口服盐酸氨基葡萄糖胶囊治疗。拈痛分子中药霜剂、双氯芬酸二乙胺乳胶剂,在痛处涂抹,每日3次;均予盐酸氨基葡萄糖胶囊口服,每次750mg,每日2次。共治疗2周。治疗前后分别采用Lysholm膝关节评分标准和西安大略和麦克马斯特大学(WOMAC)评分评价疗效,观察记录临床疗效及不良反应发生情况。结果:治疗后治疗组Kellgren-LawrenceⅠ、Ⅱ级患者的Lysholm评分均较本组治疗前增高,且均高于同期同级对照组(P<0.01)。治疗后治疗组Kellgren-LawrenceⅠ、Ⅱ级患者的WOMAC评分均较本组治疗前降低,治疗组Kellgren-LawrenceⅠ级患者的WOMAC评分低于同期同级对照组(P<0.01)。两组患者治疗期间均未出现明显不良反应。结论:拈痛分子中药霜剂联合口服盐酸氨基葡萄糖胶囊,可改善早期KOA患者的临床症状及关节功能,且操作简单,安全性较高。Objective: To explore the clinical effect of Niantong Fenzi Zhongyao Cream for treating knee osteoarthritis. Methods: Patients with knee osteoarthritis(all Kellgren-Lawrence grade Ⅰ or Ⅱ) were randomly divided into 2 groups. A total of 85 cases(98 knees) were treated with Niantong Fenzi Zhongyao Cream for external use combined with oral glucosamine hydrochloride(treatment group), and 83 cases(96 knees) were treated with diclofenac diethylamine latex combined with oral glucosamine hydrochloride(control group). Niantong Fenzi Zhongyao Cream, diclofenac diethylamine latex, daub on pain area 3 times a day for two weeks. Glucosamine hydrochloride capsules, 750 mg per time, twice a day, for 2 weeks. The Lysholm knee scoring standard and Western Ontario and McMaster University(WOMAC) score were used to evaluate the efficacy before and after treatment, and the clinical efficacy and adverse reactions were observed and recorded. Results: After the treatment, the Lysholm scores of Kellgren-Lawrence Ⅰ and Ⅱ patients in the two groups were higher than those before treatment, and the scores of Kellgren-Lawrence Ⅰ and Ⅱ patients in the treatment group were higher than those in the control group(P<0.01). After the treatment, the WOMAC Osteoarthritis Index of Kellgren-LawrenceⅠand Ⅱ patients in the two groups was lower than that before the treatment. The WOMAC Osteoarthritis Index of Kellgren-Lawrence Ⅰ patients in the treatment group was lower than that in the control group(P<0.01). There were no adverse reactions during the treatment in the two groups. Conclusion: The combination of Niantong Fenzi Zhongyao Cream and glucosamine hydrochloride can effectively relieve the clinical symptoms of early knee osteoarthritis patients, and improve the joint function. The operation is simple and the safety is high.
