壮医药线点灸结合麦粒灸治疗偏头痛的远期疗效观察

摘　　要：目的:比较壮医药线点灸结合麦粒灸疗法和单纯壮医药线点灸治疗偏头痛的远期临床疗效。方法:将符合纳入标准的90例偏头痛患者随机平均分为西药组、药线组和观察组。西药组口服氟桂利嗪胶囊治疗,20 mg/次,1次/d;药线组采用壮医药线点灸局部穴位,3壮/穴,1次/d;观察组在药线组的基础上加上麦粒灸足三里、胆俞、气海和关元穴,3壮/穴,1次/d。10 d为一疗程,连续3个疗程。分别观察治疗前、治疗1个月和治疗后6个月的VAS、VRS评分、头痛发作次数及头痛持续时间评分的变化。结果:3组远期临床疗效比较,观察组优于药线组和西药组(P<0.01)。与治疗前比较,各组治疗1个月后及治疗后6个月的VAS、VRS、头痛发作次数及头痛持续时间的评分均明显下降(P<0.01),且观察组优于药线组和西药组(P<0.05);治疗后6个月,与治疗1个月后比较观察组VAS、VRS的评分差异无统计学意义(P>0.05),而西药组和药线组的评分均较治疗1个月后升高(P<0.05)。结论:壮医药线点灸结合麦粒灸治疗偏头痛具有良好的远期临床疗效,并且优于口服盐酸氟桂利嗪及壮医药线治疗。Objective:To compare the long-term clinical efficacy between Zhuang medicated thread moxibustion combined with moxibustion with seed-sized moxa cone and single Zhuang medicated thread moxibustion in the treatment of migraine.Methods:90 migraine patients,who met the inclusion criteria,were randomly divided into the western medication group,the medicated thread group and the observation group.The western medication group was treated with oral Flunarizine Capsule(20 mg/time,once a day).The medicated thread group was treated with Zhuang medicated thread moxibustion on local acupoints(3 threads/acupoints,once a day);on which basis,the observation group was also given moxibustion with seed-sized moxa cone on Zusanli(ST36),Danshu(BL19),Qihai(RN6)and Guanyuan(RN4)(3 cones/point,once a day).The two groups were treated for three consecutive courses,with 10 days being taken as one course of treatment.The scores of VAS and VRS,migraine frequency and migraine duration were observed before the treatment,after one month of the treatment and after six months of the treatment.Results:The observation group was superior to the medicated thread group and the western medication group in terms of long-term clinical efficacy(P<0.01).The scores of VAS,VRS,migraine frequency and migraine duration significantly decreased after one month and after six months of the treatment compared to those before the treatment in the three groups(P<0.01),and the observation group was superior to the medicated thread group and the western medication group(P<0.05).There were no statistical differences in the scores of VAS and VRS between after six months of the treatment and after one month of the treatment in the observation group(P>0.05).The scores of VAS and VRS were significantly increased after six months of the treatment compared to those after one month of the treatment in the western medication group and the medicated thread group(P<0.05).Conclusion:Zhuang medicated thread moxibustion combined with moxibustion with seed-sized moxa cone has better
