摘 要:目的:观察针刀干预对II型胶原诱导关节炎(CIA)大鼠模型的治疗作用及对CIA大鼠滑膜组织中凋亡相关基因天冬氨酸特异性半胱氨酸蛋白水解酶3和9(Caspase-3和Caspase-9)表达的影响,探讨针刀干预对CIA大鼠的影响。方法:将30只SD雌性大鼠随机分为空白组、模型组和针刀组,每组10只。空白组正常饲养,模型组、针刀组分别采用免疫学方法制备CIA大鼠模型。造模成功后模型组不予任何处理,针刀组于大鼠左膝关节周围寻找条索状物行针刀干预治疗,每周治疗1次,共治疗3周。观察记录各组大鼠左后足足跖厚度及大鼠四肢关节炎指数评分(AI);运用HE染色法通过光镜观察各组大鼠膝关节病理形态学变化;采用实时荧光定量PCR法(qRT-PCR法)检测各组大鼠滑膜组织中Caspase-3、Caspase-9的mRNA表达量。结果:与空白组比较,模型组、针刀组大鼠足跖厚度及AI值均明显增加,差异具有统计学意义(P<0.01);与模型组比较,针刀组大鼠足跖厚度及AI值均明显降低,差异具有统计学意义(P<0.01)。模型组大鼠Caspase-3、Caspase-9 mRNA表达水平明显低于空白组,差异具有统计学意义(P<0.05),针刀组大鼠Caspase-3、Caspase-9 mRNA表达水平较模型组明显升高,差异具有统计学意义(P<0.05)。结论:针刀干预对CIA大鼠关节炎症有明确的治疗作用,其作用机理可能是通过促进滑膜细胞凋亡相关基因的表达抑制滑膜组织增生实现的。Objective:To observe the therapeutic effect of acupotomy therapy on rat model of type II collagen-induced arthritis(CIA)and its influence to apoptosis-related genes of Caspase-3 and Caspase-9 in synovial tissue of CIA rats,thus to explore the intervention effect of acupotomy on CIA rats.Methods:30 female SD rats were randomly divided into the blank group,the model group and the acupotomy group,with 10 rats in each group.The CIA model was prepared by the immunological method.After successful modeling,the acupotomy group was treated with acupotomy intervention around the left knee joints of the rats,once a week for a total of 3 weeks.The thickness of the left hind foot plantar and arthritis index(AI)were observed in each group.The pathological and morphological changes of the knee joints of rats were observed in each group by HE microscopy.Real-time quantitative PCR(qRT-PCR)was used to detect the expressions of Caspase-3 mRNA and Caspase-9 mRNA of the synovial tissue in each group.Results:Compared to those in the blank group,the thickness of the foot plantar and AI values of the model group and the acupotomy group were significantly increased(P<0.01);compared to those in the model group,the thickness of the foot plantar and AI value of the acupotomy group were significantly decreased(P<0.01).The expression levels of Caspase-3 mRNA and Caspase-9 mRNA were significantly lower in the model group than those in the blank group(P<0.05);the expression levels of Caspase-3 mRNA and Caspase-9 mRNA were significantly higher in the acupotomy group than those in the model group(P<0.05).Conclusion:Acupotomy intervention has a clear therapeutic effect on joint inflammation in CIA rats,and its mechanism may be achieved by inhibiting the proliferation of synovial tissue by promoting the expressions of apoptosis-related genes in synovial cells.
