剖宫产术中子宫切口双层缝合法对术后瘢痕憩室发生率与憩室容积的影响

摘　　要：目的:探讨剖宫产术中子宫切口缝合方法对术后瘢痕憩室发生率与憩室容积的影响。方法:选取2018年1月-2019年6月本院收治的200例择期剖宫产产妇。按照随机数字法分为对照组与研究组,各100例。对照组进行单层缝合法缝合,研究组进行双层缝合法缝合。比较两组憩室容积、肌层厚度、憩室发生率、围术期相关指标、月经恢复情况、生活质量、满意度。结果:研究组憩室发生率低于对照组,憩室容积小于对照组,肌层厚度高于对照组(P<0.05);两组住院时间、手术时间、肛门排气时间、恶露持续时间及术中出血量比较差异无统计学意义(P>0.05);研究组月经血量少于对照组,月经周期与经期短于对照组(P<0.05);治疗前,两组生活质量评分比较差异无统计学意义(P>0.05),治疗后,研究组生活质量评分高于对照组(P<0.05);研究组总满意度高于对照组(P<0.05)。结论:剖宫产术中选择双层缝合法缝合子宫切口,可降低瘢痕憩室发生率,减小憩室容积,提高满意度。且术后住院、手术、肛门排气、恶露持续时间明显缩短,术中出血量减少,利于恢复正常月经,有利于生活质量提高。因此,在剖宫产术中子宫切口缝合时,双层缝合法缝合值得推广应用。Objective:To investigate the influence of uterine incision double-layer suture in cesarean section on postoperative scar diverticulum incidence and diverticulum volume.Method:A total of 200 women with elective cesarean section admitted to our hospital from January 2018 to June 2019 were selected.According to random number method,they were divided into the control group and the study group,with 100 cases in each group.The control group was treated with single layer suture,and the study group was treated with double layer suture.Diverticulum volume,muscle thickness,incidence of diverticulum,perioperative related indicators,menstrual recovery,quality of life and satisfaction were compared between the two groups.Result:The incidence of diverticulum in the study group was lower than that in the control group,the volume of diverticulum was smaller than that in the control group,and the muscular thickness was higher than that in the control group (P<0.05);there were no significant differences in hospitalization time,operation time,anal exhaust time,lochia duration and intraoperative blood loss between the two groups (P>0.05);the menstrual blood volume of the study group was less than that of the control group,and the menstrual cycle and menstrual period were shorter than those of the control group (P<0.05);before treatment,there was no significant difference in quality of life scores between the two groups (P>0.05);after treatment,the quality of life scores in the study group were higher than those in the control group (P<0.05);the total satisfaction of study group was higher than that of control group (P<0.05).Conclusion:The choice of double-layer suture method in cesarean section can reduce the incidence of scar diverticulum,reduce diverticulum volume and improve satisfaction.In addition,the duration of hospitalization,operation,anal exhaust and lochia are significantly shortened,and the volume of bleeding during operation is reduced,which is conducive to the recovery of normal menstruation and the improvement of life
