摘 要:目的:探讨与分析快速现场评价(rapid on-site evaluation,ROSE)技术在呼吸道感染诊断中的应用价值。方法:将本院2020年3-12月接收的200例重症或疑难的呼吸道感染患者,随机分为两组,其中观察组使用气管镜行肺泡灌洗获取标本后利用ROSE技术快速评价标本;对照组则单纯使用气管镜行肺泡灌洗获取标本。最后两组标本均送细菌培养及药敏。对比两组患者细菌阳性检出率差异、标本检测周期、气管镜操作时间、气管镜操作次数、气管镜操作所致并发症发生率(包括出血、气胸等)等情况。结果:观察组灌洗液革兰菌阳性检出率高于对照组,且检测周期短于对照组,差异有统计学意义(P<0.05)。气管镜操作时间、次数及气管镜操作所致并发症发生率与对照组比较,差异无统计学意义(P>0.05)。结论:在支气管镜下获取肺泡灌洗液后采用ROSE技术评价标本,具有检测周期短的优势,且不增加气管镜操作的时间、次数及并发症,可快速为临床提供初始抗感染治疗的依据,从而指导治疗过程中的精准用药,临床价值较高,值得推广使用。Objective:To discuss and analyze the application value of rapid on-site evaluation(ROSE)in the diagnosis of respiratory infection.Method:A total of 200 patients of respiratory infection admitted to our hospital from March to December 2020 were randomly divided into two groups.In the observation group,samples were obtained by bronchoscope alveolar lavage and then rapidly evaluated by ROSE technology.In the control group,samples were obtained by bronchoscope only.All of specimens were sent for bacterial culture and drug sensitivity.Finally,the difference in positive bacterial detection rate,specimen detection period,operation time of bronchoscope,operation times of bronchoscope,incidence of complications caused by bronchoscope(including bleeding,pneumothorax,etc.)were compared between the two groups.Result:The positive detection rate of the observation group was higher than that of the control group,and the detection period was shorter than that of the control group(P<0.05).The operation time,frequency and the incidence of complications caused by bronchoscope in the observation group were compared with the control group,and there were no significant differences between the two groups(P>0.05).Conclusion:Alveolar lavage fluid evaluated by ROSE has short detection period,and does not increase the operation time,frequency and the incidence of complications.ROSE can guide the precision in the process of medical treatment,promote the recovery and prognosis of the patients.ROSE has high clinical value and worthy promoting.
