参麻平肝益智汤加减联合银杏叶片对肝阳上亢型血管性痴呆患者MMSE评分及脑血灌注情况的影响

摘　　要：目的:观察参麻平肝益智汤加减联合银杏叶片治疗肝阳上亢型血管性痴呆(VD)的效果及对患者简易精神状态评价量表(MMSE)评分、脑血灌注情况的影响。方法:选取2018年6月~2020年9月在本院接受治疗的肝阳上亢型VD患者114例,采用随机数表法分为观察组和对照组,均有患者57例。其中观察组患者采用参麻平肝益智汤加减联合银杏叶片治疗,对照组患者仅采用银杏叶片治疗。观察两组的治疗疗效和中医证候改善情况,对比治疗前后两组患者脑血流灌注情况、MMSE、神经功能缺损评分(NIHSS)、日常生活能力量表(ADL)评分以及炎性因子浓度变化。结果:观察组中总有效率高于对照组(P<0.05);治疗后,观察组患者的脑血流灌注参数双侧大脑动脉(MCA)、后动脉(PCA)、基底动脉(BA)均大于对照组(均P<0.05),MMSE、NIHSS、ADL、各项中医证候评分以及血清炎性因子白细胞介素-6(Interleukin-6 IL-6)、血清肿瘤坏死因子(TNF-ɑ)浓度均小于对照组(均P<0.05)。结论:参麻平肝益智汤加减联合银杏叶片可显著改善肝阳上亢型VD患者的脑血流灌注状况和神经功能,改善其炎症反应、提高患者的日常生活能力,值得在临床治疗中推广。Objective: To explore the curative effect of Modified Shenma Pinggan Yizhi Decoction combined with Ginkgo Leaf Pills on vascular dementia(VD) of liver-Yang hyperactivity, and its influences on score of mini-mental state examination(MMSE) and cerebral blood perfusion. Methods: A total of 114 VD patients of liver-Yang hyperactivity who were treated in the hospital from June 2018 to September 2020 were enrolled and divided into an observation group and a controlled group by the random number table method, 57 cases in each group. The observation group was treated with Modified Shenma Pinggan Yizhi Decoction and with Ginkgo Leaf Pills, while the controlled group was only treated with Ginkgo Leaf Pills. The curative effect and improvement of TCM syndromes in both groups were observed. The changes in cerebral blood perfusion, scores of MMSE,National Institutes of Health Stroke Scale(NIHSS) and Activity of Daily Living Scale(ADL),and concentrations of inflammatory factors before and after treatment were compared between the two groups. Results: The total response rate in the observation group was higher than that in the controlled group(P<0.05). After treatment, cerebral blood perfusion parameters [middle cerebral artery(MCA),posterior cerebral artery(PCA),basilar artery(BA)] in the observation group were higher than those in the controlled group(P<0.05),while the scores of MMSE,NIHSS,ADL and TCM syndromes, and concentrations of serum inflammatory factors [interleukin-6(IL-6),tumor necrosis factor(TNF-ɑ)] were lower than those in the controlled group(P<0.05). Conclusion: Modified Shenma Pinggan Yizhi Decoction combined with Ginkgo Leaf Pills can significantly improve cerebral blood perfusion, nerve function, inflammatory response and activity of daily living in VD patients of liver-Yang hyperactivity.
